hanks I’m gonna give it my best shot, no pun intended. See I didn’t realize the protein could improve with regulation, good to know.
Well, if you look at the causes of protein in the urine, one of the things usually listed is feline diabetes. Now, if the kidneys are damaged then it may not be reversible. Unless you do the UPC Ratio testing you may not find out the situation. Right now, I'd say the first priority is to get the diabetes under better control. You are well on your way.

Oh, and about the spreadsheet, it's very important to keep your spreadsheet updated pretty much with every test. If you have a situation where you need dosing advice or your cat is dropping too low, then the people on this Board who are helping you will need to be able to quickly look at your spreadsheet to see what's been happening. It would not be good if you are asked to update your spreadsheet an there's a delay while you input a week's worth of data. Does this make sense? And without data, we're not really allowed to comment.

Anyone have any tips when first getting started with the testing and shots and dose? That’s the only thing I haven’t researched yet, next few days.
We need to get you set up with a spreadsheet in googledocs. And a signature with information about Abbott.
Have you read about and chosen a dosing method yet? I'm going to link the document below that describes the two dosing methods. It really depends on how much testing you will be able to do as to which method you will want to begin with. Have you ever done a BG ear test yet?

Yep, I mentioned in my last update that i have successfully given ONE bg test. lol. I have all the testing supplies, and hypo stuff. I think he will be a good tester, it went well.
I just got the Lantus solostar pen today so hopefully give my first shot tonight.
I am reading up on the dosing stuff you linked.
I already have a good handle on testing and shooting stickies.

I have updated signature. Since his BG isnt super high and no history of ketones I am going with the SLGS method. Still working on the spreadsheet. I am using the creating a spreadsheet sticky but I want to make sure I set it up well and don't have any of the issues other members have had with the "view only" thing. Any tips are welcome.

Trying to zero in on a SLGS starting dose, his test yesterday was 300mg, he's been on a LC wet for years. So i'm thinking 0.5?
Of course I will make sure to do a PMPS before his very first dose.
Also, have you used the pen? Does it go back in the fridge after opening? I've read the stickies on that and some say it shouldnt go back in because a pressure thing but seems not likely to last as long if outside of fridge.

Yep. .5 would be your starting dose and hold for a week unless a reduction is earned. With a ReliOn meter he would earn a reduction if he drops under 50.

OK, on the sticky it says to hold starting dose for a week and then do a curve after 1 week.

Hold the dose for at least a week:
  • Unless your cat won’t eat or you suspect hypoglycemia
  • Unless your kitty falls below 90 mg/dL (5 mmol/L). If kitty falls below 90 mg/dL (5 mmol/L) decrease the dose by 0.25 unit immediately.

But how often should i be testing during that first week?
What kind of BG values should I expect during first week?

Hi Drew. I saw your message in that private conversation that you were tagged in. I didn't mix you up. You were supposed to start at .5 units. When will you begin? You are probably ready. You are one of the most well-informed people to begin with insulin. You've read so much on this Board. I am proud of you!

How frequently will you be able to test, do you think? I would try to get tests at different hours... +2, +4, etc. on different days... both in the a.m. and p.m. cycles. A lot of cats tend to drop lower in the evening cycles, so getting at least a +2 or 3 test before bed is always helpful. If one of those tests is a lot lower than the PMPS number, then you may have an "active cycle" (which means you need to stay up with your kitty! lol! or set alarms to wake you up to test to make sure Abbott is in safe numbers.)

Ok, still confused about the +2, +3, etc.
Can you just walk me through this first day or two please?
I should be able to test maybe 4 times a day.

So, say like PMPS test at 8pm, then feed say at, wait 30 min shoot, test again at 11pm. would that be a +3?

If you feed and shoot at 8 p.m. then a +3 would be 11 p.m. We try to test, feed and shoot all within about 10-15 minutes. Just get your fasting BG test (no food for 2 hours prior to the pre-shot test) and feed Abbott and then shoot the insulin (some people even do this while they are eating.)

With Lantus, you don't need to wait 30 minutes after eating to go ahead and shoot the insulin. As long as he's eating, it will be fine. He's not prone to vomiting after breakfast/dinner, is he? Is he a good eater?

Ask any and all questions! I’m impressed with your knowledge already. Abbott is lucky to have you. We will help you in any way that we can!

Labs are similar to last time, yes. I would work on getting his diabetes under better regulation and then see if some of those labs (including the protein in the urine) don’t improve.
@Suzanne & Darcy
Hey! This was his last round of testing. I’m looking into getting him checked out. Had a few questions for you.

1. He’s been tightly regulated now for a couple weeks, too soon to run lab work?
2. What tests would you recommend besides CBC, superchem, t4, urinalysis? Still recommend UPC ratio?

Yes CBC, superchem, with T4. Urinalysis will show if he’s got protein in his urine. Ask the vet if it’s significant enough to send out the urine for the UPC ratio test. They should be able to use the same sample. Some vets want multiple samples over several weeks. Hopefully this is not the case.

I would say it’s not too soon to get labs done — but Maybe you should wait until after Labor Day (about 3 months from the last labs).
Thanks, couldn’t remember about the UPC thing but that clears it up. So 3 months is usually when you’d see changes from regulation? Basically I just don’t wanna test to soon lol.
