The good (relatively) numbers and then drifting upwards happened to us recently, Tommy, and I suspect it was the insulin going bad because it was after 4 weeks. I got the new vial going just the other day and it seems to help? I just wish I could squeeze out that last drop because it's expensive, but it deteriorates and loses potency over time. My last idea to help keep it stable over time is to put a small glass over the vial in the fridge so it has a micro climate in there.
About the dosing, if he is bouncing I've been reducing and the numbers level out better, although they are a little higher than I would like (mid 200s to 300 range?) I think it's better than the rollercoaster ride.
