? Theodore striggling 7/27 +1 430

Hello,

I am really struggling with Theodore's UPC. His best UPC was 02/12/24 at 2.7. His last UPC which was recent was 3.8. All really high. He's been on telmisartan and Benza. All no results and make him out of it. I took him off. Only advice vet advised was to "change food to kidney food", we did that and it caused EXPLOSIVE diarrhea. Yesterday it happened and he peed on the floor, this morning too. He's a little out of it and lost weight down to 13.2. Any help would be appreciated, I don't know what else I can do. He also still has the one mycoplasma (Candadatus)---could this be causing issues?

I have no experience with mycoplasma. - have you asked the internal medicine vet your questions?

I also have no experience with treating proteinuria with telmisartan. Neko was taking benazapril for it. When her UPC went higher, the vet upped the dose.

Did the vet say why the change to kidney food? Was the thought that he needed less protein in his diet? If so, try looking for a commercial food that is higher in fat and lower in protein than what he was eating. That might help the weight loss too.

I have him on DM right now as it helps him when he has diarrhea. It’s been happening last two days. I’m wondering if this proteinuria is IBD related inflammation. I know steroids aren’t great for cats but would it be wrong to ask? The other benefit it would help his RBC’s and weight gain

I've never heard of proteinuria being related to IBD. It has to do with the kidneys not able to filter out protein from the urine. IBD doesn't cause excess protein. What would the steroids be for? If IBD and it's been confirmed that he has IBD, please put that in the signature. An alternative steroid for the GI system is budesonide, which often doesn't impact blood sugars. It had no impact on Neko's BGs.

Did you talk to the vet about trying benazapril instead?
 
IBD has not been confirmed, I was just thinking maybe that's what it is as he seems to be sensitive. Doctor doesn't' want to give him steriods, we just spoke this morning given he's immune comprised with FIV FELV she says theres more risk..
 
That makes total sense - steroids can lower their immunity more, so not a good idea.
Thank you @Wendy&Neko . We are going to try another round of antibiotics (Veraflox) since he still has 1/3 mycoplasma. Although it does bring on diarrhea, the DM does help him bulk up his poops...I am hoping it's just the mycoplasma that was lingering which may need further treatment but I am really starting to question his quality of life. :(
