Switching from Vetsulin to Lantus

What's the best approach for switching to a different insulin?

Should I not shot for a day (at this point, probably won't make his numbers worse LOL) or continue life as usual and just go for it?

My vet agreed to switch Gordy to Lantus to see if it goes better (YAY!) so I'll be picking it up Saturday and starting it Sunday. We'll be doing 1U and I think I'll do the SLGS method, at least until I get him closer to being regulated.

(Should I point this in the Lantus page? I figured a lot of people here have experience changing insulins.)
 
Hi Cass,

Switching from Vetsulin to Lantus is fairly straightforward in principle. Vetsulin is an in-out insulin so the previous dose should be out of the system by the time the first Lantus dose is due. If I were switching my cat I would want to be able to keep a close eye on BG levels so it's great that you'll be able to keep an eye on Gordy on Sunday.

Note: it is possible to sometimes see a slight influence on BG numbers in the cycle following a Vetsulin dose (a phenomenon referred to as carryover) but typically it's not significant, though extra caution should be taken in cats whose BG levels have recently been running in relatively good numbers.

For general info, going from Lantus to Vetsulin could well be a trickier switch because one would still need to take into account any Lantus still in the depot for the first few cycles after the changeover.

I think it's a good idea for you to post on the Lantus & Lev board prior to the switch. They may have additional, more specific advice to give you - especially about the Lantus starting dose.

For cats who are just starting insulin treatment the cat's weight and body condition are used in order to work out a starting dose. However for a cat who has been receiving another insulin prior to starting Lantus the dose and BG-lowering effect of the previous insulin also needs to be taken into account when determining a suitable starting dose.
