Should I give him insulin ?

My fear with shooting him is that he was misdiagnosed they never did a fructosomine test on him and he’s only tested over 400 once (he also had a flare up of pancreatitis at the time) he tested 211 again tonight after a large meal for his metronidazole less than 30 minutes ago and his dexamethasone injection was an hour ago I know dexamethasone also raises blood sugar and can cause diabetes since he tested 400 during his pancreatitis flare (also at the vet in stress) we’ve lowered the dex a decent amount he hasn’t been over anything 211 since should I still shoot that ? I know I messed up with the feeding tonight so this question is for learning purposes I will not shoot tonight

The normal BG for a cat on a human meter is 50-120. It would be higher on an AT and normal seems to vary by vet but my vet uses 68-150 on an AT outside of the vet’s office where it’s likely lower. I’ve seen spikes up to high 100s on an AT, due to vet stress, in a non-diabetic cat.

What I’d suggest is trying to find out what his BG is when he hadn’t been fed for two hours and he’s further away from his dex dose. But because dex can cause diabetes, sometimes temporary and sometimes permanent, he might need some lower dose insulin support until he’s weaning off the dex. That should be discussed with your vet. Has he given you a BG above which you should shoot?

The bottom line is I’m not a vet and you’ve been working with them on his special needs for quite a time. While I might give my own cat insulin in a similar situation, with all Ziggy’s comorbidities and because you’ve finally gotten him stable, I don’t want to undermine your vet’s advice.

In other words, think about discussing it with him again especially if you decide to start taking the dex dose down.

I’m sorry I was sleeping yes I understand completely what you mean , I was told I should test after dex and not shoot below 250 if my mid day test is normal. When I test Ziggy with no food or dex he ranges from 90-146 throughout the day. Once we lowered his dexamethasone on the 16th after diagnosis he has tested much lower. I believe as well there was a steroid induced diabetics issue we caught early. Thank you for having me on the forum because with his extensive issues I want to do whatever I can to keep him healthy and happy ! I’ll trying to get better at the readings and offer more information on SS

Hey everyone so today we got our fructosomine test results in and the doctor is now saying he is not diabetic but possibly developing diabetes any and all suggestions welcome he already has so many issues if there’s anything I can do to help him please let me know

You need to continue to feed a low carb diet for the rest of his life.
I would keep getting the twice daily BG tests in for a few weeks so we can see what’s happening.

My vet said she would email me the results but I haven’t gotten them yet I’ll share them as soon as I receive them I promise ! I tried to test when I got home today but the Bg monitor kept saying add more blood and I got a lot ! He has another vet appointment tomorrow not our primary but a brand new IM specialist so I’ll have them run an in house glucose test in the morning also and try to get to the bottom of whatever is going on with ours his new food came in today they rayne nutrition kangaroo and we’re gonna begin the transition slowly after the vet tomorrow he has to fast for some tests he also came up with abnormal in house pancreas testing so we’re gonna investigate that further also

That result is within the normal range.

so it looks like they were too quick to diagnose him I’m still going to monitor bg at home.. developing is still a fear for me I can’t believe he was given a script for insulin and we were instructed to blindly shoot every 12 hours thank goodness for this group because I would imagine if he isn’t truly diabetic at the moment that would have been a big problem ?now we’ve reduced his dexamethasone shot down and are continuing to transition onto rayne nutrition kangaroo regardless.. on top of FIV and IBD he has pancreatitis and (possibly)EPI, his Flp snap test showed an abnormal pancreas which hints at both his ultrasound showed inflammation in the pancreas as well he wasn’t fasted for the EPI test we go back for that Monday he also apparently has blood parasites called m. haemofelis and m. haemominutum it’s been a roller coaster may I still please stay in this group to read and learn even though my baby doesn’t have diabetes as of now it’s still a concern moving forward and I would love to learn everything I can because as you can see I’m not working with great doctors all the tests that I got answers from I had to look up and figure out how the dots connected and what tests to do.. is it wrong that I’m upset they diagnosed him diabetic without a fructosomine test and if it wasn’t for this group I wouldn’t have known that had I not looked into it further I would have been giving my kitty insulin for almost a week could that have caused problems? after everything I’ve read I know there is a very good chance he could become diabetic I see it happening on my IBD kitties page after bouts of pancreatitis which he has so please say a prayer for my baby with FIV IBD pancreatitis EPI (I’m almost certain about the EPI) and two blood parasites were living on a prayer thank you all so much for helping us you all saved my baby with your knowledge because had I blindly followed the vet who knows where my baby would be !

What did the IM vet say?

IM said his ultrasound looked good over all all things IBD concerned his GI looks quiet his pancreas is inflamed however and the in house snap Flp test showed abnormalities in his pancreas he also has two types of uncommon blood parasites he kept testing low for all the RBCs I told her run every test you have and that’s what she found so on he’s gonna go on antibiotics for that and we’re doing an EPI test Monday (he has to be 12 hours fasted for that) I’m convinced he has EPI also
 
You all already have helped me so much I don’t know what I would do without you guys thank goodness for people like you I wish more people cared about their animals the way people in these groups do IBD kitties have saved Ziggy more than the doctors have multiple diagnosis kittys are really tough and he’s so young I know we can’t stop or change the shot but I am looking to reduce it once we make a full switch to the new food too many things at once with Ziggy send him into an IBD flare and that’s a week minimum in the ER and it’s so hard on him so my plan is to slowly switch food this food he is on is 37% carbs the one he’s being switched too is 14% I’ll slowly switch that and if it doesn’t cause a flare by time he’s fully off the I/d dry I’ll wait two weeks and lower the steroid .5 with ultrasound monitor to see how his GI handles it
I agree with your entire quote! The people here are AMAZING. You are also doing so much for ziggy, he is a lucky guy! I wanted to send you support on solo blood testing. It is SO hard in the beginning. My hands would visibly shake. It gets better. Best of luck to you

Thank you so very much oh how my hands shake he’s got so much going on and he has been in and out of vets all week we’re still monitoring at home just less frequently since his fructosomine test showed he’s not diabetic but I do know since he’s on cortisteriods for life it’s a possibility so I’m gonna continue to monitor so I can keep it at bay hopefully he’s getting a break for a few days he has so much going on a rare blood parasite, pancreatitis FIV, IBD and an infection on his paw pad and that needed surgery this Saturday we go back for recheck of the paw tomorrow with all the medications and the shots I’ve given him the last two days off from the ear pricks my poor baby has so much going on but I’ll get his BG at the vet tomorrow also with all the new medications and our recent normal fructosomine test I’m just trying to give my poor baby a break he’s had a rough two weeks thank you so much for your support and your care we have begun transitioning his diet to a food formula for IBD pancreatitis and diabetes rayne nutrition kangaroo we also lowered his dex shot so hopefully all that makes a difference he’ll be on low carb food for the rest of his life and luckily he loves it !

He definitely has a lot going on and you are obviously doing your very best. Make sure you are being kind to yourself and give ziggy an extra hug for me. Ps the food is often half the battle! Go ziggy!

thank you so very much everyone in this group is so kind and supportive I will give him a whole bunch of love and hugs from you and everyone who’s helped him along the way ! He’s my whole world I’m gonna get him through this paws crossed this food does well for him

Hey everyone Ziggy here again I’m unsure how to start a new post so I’m just going to try and continue here. Ziggys had several fructosomine tests to monitor for diabetes not only have they been low but they have shown improvement. However his IM believe that there is too much sugar in the urine and wants to start him on a low dose of insulin just to be safe. He said he’s not concerned about BG just the sugar in the urine does this make any sense ?
