Help with dosage

Hello FDMB community,
I'm new here and could really use some advice on my diabetic cat, Puffy. He's a 5.75 kg male, diagnosed with diabetes about 13 months ago. He's on Lantus insulin, injected every 12 hours at 9 AM and 9 PM. His diet is strict raw BARF low-carb: mostly raw meat, hearts, organs (heart, liver), taurine supplement, vitamin B12, inactive yeast, boiled egg yolk, kefir (minimal, about 1 tsp/day), and salmon oil. No carbs over 5-6%,
. He's been on Stress Control supplement (1/2 capsule/day) for anti-cortisol for about 10 days, and seems fine with it.
His blood glucose has been unstable lately with a yo-yo cycle: low mornings (even hipo like 61), skip dose, high evenings (400+), then back to 5U, and repeat. I use Accu-Chek Instant meter. Here's his recent spreadsheet with values, doses, and notes:
Google Sheets Link: Puffy's BG Spreadsheet

I'm worried about the cycle and how to stabilize him. Should I stay on 5U BID? Or increase to 6U to break the yo-yo? Any advice on doses, infections, or remissi on chances after 13 months? Thanks so much – really appreciate the help!

It doesn't look like you linked your spreadsheet. You need to open the icon that looks like a paperclip, paste in the the information from the top of your browser that has the location of the spreadsheet, then click on "insert."

We do not increase doses in 1.0u amounts. We typically make dose adjustment in 0.25u increments. By increasing in a large amount, you an go flying by what would be an effective dose for your kitty. Also, most experienced members would be hesitant to give you dose advice without benefit of seeing your spreadsheet. We want be be sure to keep Puffy safe.

@danowics it doesn't look like your link to your spreadsheet is active in this posting, although it is in your more recent post.
Please just add Puffy's spreadsheet to your signature, so the advisors here can see how your cat is doing, and help you with insulin dosage and more.

Here's how to add the information we need to your signature.

Make sure your signature is up-to-date.
  • In the top right, click on your username, and then Signature. This is where you will put information that helps us give you feedback. There is a limit of two lines which may include two links; you may separate pieces with commas, dashes, | etc. This is where you paste the link for your spreadsheet, once it is set up.
  • Add info we need to help you:
    • Caregiver & kitty's name
    • DX: Date
    • Name of Insulin (do not include dose or frequency)
    • Name of your meter
    • Diet: "LC wet" or "dry food" or "combo"
    • Dosing: TR or SLGS or Custom (if applicable)
    • DKA or other recent health issue (if applicable)
    • Bexacat or Senvelgo (if applicable) and dates
    • Acro, IAA, or Cushings (if applicable)
    • Spreadsheet link. Please put the signature link on the bottom line of your signature information, on its own, so it is easy to find.
    • Please do not put any information about your location in the signature for security reasons. If you wish to add your country location, please add it to your profile.
Be sure to click the 'Save Changes' button at the bottom. If you need help urgently it is important we know these things at a glance. We don’t want to waste valuable time finding out information.
