Ginny's Nadir

Good evening all (from NSW Australia)

I'm still a newbie (June Diagnosis). Ginny is doing well. We've been plodding along with BG tests, LC meals, and insulin, and I can see it's all working. I did another curve today, and I can see the bigger picture of improvement (her results seem to be stabilising around the yellows, which will soon hopefully introduce her body to more blues as the 'norm'). However, I'm still having trouble understanding the nuances, like Ginny's nadir, which I had thought was around 8+ mark, but today's curve (date: 26-7) seems to show no nadir at all? The lowest points of today's curve seems to be the AMPS and PMPS.

10.5 17.4 15.8 14.9 14.8 14.9 13.6 13.2 13.1 13.2 11.7 mmol/L
189 313 284 268 266 268 245 238 236 238 211 mg/dl

I think I need several more curves before I can make any real conclusions but I thought I'd share and see what the community here thinks.

Thanks, guys

hi Kimmy and Ginny, hello from the states! My wife is from QLD, I love Australia.

I see what you mean about the cycle, pretty flat with no clear nadir. Flat is good though! Nice and stable without big ups and downs is good progress.

do you post on the Lantus/biosimilars forum at all? I ask because it looks like you've held a dose of 1.0 U for about a month and normally people choose a dosing method (SLGS or TR) and post daily threads (called condos) on the Lantus forum for help in dosing. This is what helped me get my cat Hendrick so well-regulated. I highly recommend it!

Hi Kimmy,
Good to hear from you …also from NSW….
Are you still testing for ketones? I can’t see any results in the SS at all.
I would suggest testing at least once a week at this stage given the fairly recent history of DKA.
How is Ginny’s appetite?

As. Kyle has suggested…have you chosen a dosing method yet…it would be a good idea to follow one of them as it gives you structure and a road to follow DOSING METHODS
I would also recommend you start posting over on the Lantus forum each day and you will make quicker progress.
I would suggest increasing the dose to 1.25 units and try and get at least one test in during every cycle to see how low the dose is taking Ginny.
I know you won’t have the 1/2 unit syringes but I used to eyeball the 1/4 unit doses. I used a magnifying glass and a good light to help me.

Hello again, @Bron and Sheba! We're actually only an hour away from you - we live on the central coast, Wyoming. :cat:

Ginny's appetite is quite lovely; she's satisfied after mealtimes, no breaking into bread bags as it seems sugarcats are wont to do, but she's also appropriately hungry in the lead up to meal times, so she's doing well.
Having read some of the information on the dosing methods, I'll admit, I'm not quite certain. I think the SLGS methods appeals to me more, because I don't mind being patient, and I don't feel like I fully understand all the effects of insulin and food on Ginny's SS just yet so I'd rather just go by a weekly curve, steady, wait and see route. I realise, however, now, given yours, Kyle's, and Diane Tyler's Mom's advice, that Ginny is due for an insulin increase as her nadir is consistently above 8.3 mmol/L (150), and I'm on board. I'm just not comfortable yet about trying that tomorrow, or at least before I get syringes with half marks. I've heard people use something called a caliper, maybe I'll get one of those?

I haven't posted the results for any ketone tests in the SS because I don't have them - the vet only told me that each time they've checked her urine, she's had no ketones. They don't give me the actual results. Ginny is due, however, for another urine test at the vet. I haven't ordered a ketone blood meter yet - I plan on doing this on the weekend. I will also try and order some 1/2 unit syringes, because I feel nervous enough measuring out a 1u with a marked line in the syringe without trying to eyeball doses in between lines :nailbiting:

I'll have a go at posting on the Lantus forum tomorrow morning just to get to know the room and see if I can't get started with Ginny's new dosing method instructions. Thank you to you all for the care and attention, advice and information <3 :joyful:

Kimmy you can’t buy syringes with 1/2 unit marking in Australia unfortunately. We used to be able to buy them from the UK until Covid but they have stopped posting to us now. I will have a look online and see if I can find another overseas place that will send to Australia. You could try callipers.

I would strongly recommend you buy some Ketostix from a pharmacy and start to test for ketones yourself. After DKA it’s really important that they are tested daily for a few weeks as any sign of ketones needs to be dealt with immediately.

I did not use syringes with 1/2 unit markings on them at all except when someone sent me some auto try. I used to eyeball the dose using a magnifying glass and a good light. I d not leave it too long before increasing the dose to 1.25 units
