Dosing Help - Big George is Home and first dose by myself

At +6 hours his BG is now 193! He probably had another tsp of HC while trying to test so I guess that will make it spike more in a few hours?
 
Good morning. I see you had some help last night, for which I am very grateful. What is your time zone and what will your shot times be going forward? Or is there a shot time that you will be working your way back toward?

Marje’s suggestion of 2 units was exactly what I was thinking (I saw the thread just before I was about to go to sleep and I saw that Marje had come along to help 😊). The numbers, though, that you had last night give me great pause about the 4 unit dose (or even a 3.75 unit dose.) I am convinced that he is being overdosed? What time (and time zone) will he be ready for his shot this morning?
 
I am grateful for all of the help and support! Marje’s suggestion was to not go earlier than +10 which would be 8:30, I think! Shot was supposed to be at 6pm, didn’t give until 10:24pm! I’m in CST in Middle TN!

Do I need to test him again now or okay to go 2 hours?

I’m also trying to do my spreadsheet but working backwards with no sleep is slow going right now but I know very important for him!

Re the spreadsheet… you need to start at the top with the first day of testing and then on the day after is the line below. You have it back to front.

No HC since 4:40! I will change the spreadsheet! I thought someone said the other way! I still have a day or two to add as well! He’s had some licks of LC in trying to get this test done!
 
He’s bouncing from the lower numbers which he is probably not used to. This is normal so don’t worry about the high numbers.
Only feed low carb food now and no food for the 2 hours before the next dose of insulin.
I’m off to bed now. I’m sure @Suzanne & Darcy will be along soon to help you with the next dose😀
 
Thank you! I think I’ll have to push the dose another 30 mins since he did have a LC snack with the last blood draw!

Now that I’ve seen how hard you had to work, I agree with Suzanne that 3.75u is too much. The challenge is getting close to a dose that isn’t too high for him but is enough to ward off the ketones.

PZ doesn’t have a depot like Lantus although it can have a bit of a residual effect from a higher dose. I’m not convinced that 2u caused the drop.
He saw a bit of blue on 2u of Vetsulin and then it looks like he bounced and last night he cleared the bounce which caused the fast action. I’m thinking his dose should be 2.5 to 3 units. Let’s see what @Suzanne & Darcy thinks.

Also, I didn’t realize his shot was over four hours late so we will have to walk that back to whatever time you want to regularly shoot.

Suzanne, since PZ doesn’t have effects like Lantus, she should be able to shoot at +10 or do you think +11 is better? I know advanced members using PZ can shoot every 8 hours which I’ve only seen one person do with Lantus due to the depot. My point being we have a little more latitude with PZ.

Steph, I’m sorry for the extra work on the SS. I wasn’t clear but I meant the data we needed most was the recent. I didn’t realize there wasn’t much of it but what I meant, since he had been dx on the 22nd, was to drop down six lines and put in the 27th data and then move up a line from there with the data from the 26th and work backwards so the 22nd was at the top. It was late here, you were having a hard time getting blood, and I was really worried about leaving you alone. It’s not something I would ever do or have done. I apologize for your extra work when you were tired. Thank you for redoing it.

Of course! I figured you were super sleep deprived. I've accidentally put my phone in the fridge with the Lantus before in a haze of sleep deprivation!

Based on what little I've seen between your dedication and the thoughtful + experienced advisors here - I know you'll get it all figured out :)

Getting blood hasn’t gotten any easier as he is angry with us! Thank goodness he doesn’t have teeth! He had a LC snack with the +8.5 reading so I think I would need to give the next dose at +10.5 or if we think safer +11 which are coming up!

I’d go with +11, all things considered. I reached out to Suzanne and hope she will also give some input on the dose but if she doesn’t, I’d go for 2.5u. He’s going to bounce several cycles but we can adjust upward if we need to. Pls check ketones every day.

Since you just gave the shot, and that’s a good thing, get a +1 and if you can’t then definitely get a +2 and make sure he eats a little food at that point. Okay?
 
Let us know about the ketones. Remember: food, fluids (hydration) and insulin are all very important.

Did the vet say what brought on the DKA? Infection? I never did find out what kind of infection caused my hat to go into DKA. He had a fever of 104.5 when I took him to the ER.

They didn’t but my GP vet thought it was probably his teeth! Also, not sure if it was just the time waiting of him going to the vet, getting the results back and then being hesitant to start the Bexacat!

.8 is pretty good. Feed him a little more at +3 and +4 or if he wants food feed him in small amounts at a time.

When he’s high, yes, stick with LC. To keep ketones at bay, you want to feed a little more calories than he’s used to and keep him well-hydrated. Did the vet take a good look at his mouth to see if it looked like there was an issue? Often it’s hard to see but if he has a problematic tooth, rubbing a Qtip on it might cause him to chatter.

Please be sure and let us know right away if he starts eating less, isn’t drinking, is lethargic, etc.

ADW has a variety of U-40 syringes with half-unit markings. I’m not sure if you can get them at Walmart but just be sure you are buying U-40 insulin syringes.

Edited to add: a bit of housekeeping on the SS when you can, please.

Okay, good to know to keep letting him eat some! I don’t know if they did at the ER! I think that my vet took a look but idk how good of a look! I can try the Qtip!

He is eating fine but I haven’t seen him drink water and he’s sleeping but wakes up every time we go to see him so I would say he’s lethargic!

You’re welcome. Just keep an eye on his ketones. He might be tired from fighting the tests last night.

It’s best if the vet looks closer at his mouth next time he’s there. They know exactly where to touch along the tooth and gum to see if there is pain.

In the PMPS cell, you will “stack” it:
116 @ 13.5; RT 117; 112 @ +15; NT @ + 16.5

RT is what we use for “retest” and “NT” is “no test. But with 2u in the Units column after PMPS, we will know you didn’t test but you shot.

Yes, hold the 2.5u dose unless he’s dropped below 150. If he’s 150 or below, don’t feed him and post right away so we get on it immediately so you don’t have to stall long.

If he’s over 300, you could shoot at +11 tonight. Otherwise, I would shoot at +12 from this morning as we only want to move the time back once a day unless he’s really high.

If you shoot at +12 tonight, you can shoot at +11 again tomorrow morning provided he’s above 200.

Does all that make sense?
