Just a reminder: pancreatitis is too high to register on the reference range.
Blood gas pH looked good at 7.376
Ketones fluctuate but remain high at 3.9 (5/8/26) on Keto Mojo. Vet asked me to switch to the Precision Xtra
He had a better day today. He always seems worse at night.
I am trying to add meds one at a time since he is sensitive and I will want to know if and what he reacts to. He is tolerating Cerenia and Zofran . He got 80 mLs of subQ fluids. He has a very good appetite. He has stopped eating his novel protein pork diet. I am boiling chicken and baking lean pork lean so it’s hard to measure the calories since it’s not canned cat food. The chicken is risky as it’s not a novel protein (IBD), but he IS eating.
1. He lost weight again. He wasn’t big to begin with. He was diagnosed at only 9 pounds (he was the runt of his litter). Today he is 8.22! :( The vets haven’t done a Rx for A/d or recovery. They are worried it will cause further GI concern.
2. He now has a Board certified nutritionist who is trying to transition him to rabbit. Rabbit-MAINT Pate The carbs is 8.84% DM but it has potato, potato starch and dextrose. Won’t those increase his glucose?
3. I am really struggling with the libre. Two misfired yesterday, every spot I pick on him is either bone or skin folds except his scruff and I want to reserve that for fluids. He is hard to test because he just doesn’t sit for it or like his ears touched. When do you want me to test? AMPS, +3, +6, +9? Other?

The potato will increase his carbs. Check out Stella & Chewys. They have a raw dried rabbit morsel. My guys (they are not diabetic) love it. ZiwiPeak. has a rabbit and lamb combination. There are also other brands of rabbit that are low carb.

I'm tagging @Wendy&Neko and @Marje and Gracie . They've had more experience with giving fluids than I have. I suspect you'll be fine if you give fluids on the opposite side of the scruff from where you give insulin.
 
Koha has a LID rabbit that is low carb and good for GI issues / food allergies / etc. Rabbit Cat Food: Limited Ingredient Diet Rabbit Au Jus for Cats

Rawz is another good one: 96% Rabbit Pate Cat Food - RAWZ

@Staci & Ivy can help with the Libre issue. Have you seen the tips here? Getting Started With Continuous Glucose Monitors (CGMs) for Diabetic Cats

You always want to get a AMPS and PMPS. The in between tests are up to you but try to get at least one or two during a cycle if you can. Switch up the times every other time to get an idea of how your cat responds. Mini curves and full curves can be done when you have a full day free.
 
i can’t remember if any of us linked this post for you on how to give subq fluids at home. My DH and I filmed it with step by step tips which really make a difference.

You want to give subq fluids away from where you shot. You can do them in the opposite side scruff but there is a large area where you can give fluids (shown on the video).

Keep in mind that subq fluids may drop the BG a bit in “some” cats. It never did in my Gracie but I have seen it do so in several members’ cats.

One note about the test for pancreatitis: the number doesn’t relate to severity. The higher the number, the higher the probability that it is pancreatitis but diagnosing pancreatitis should be done by ultrasound in conjunction with superchem labs and the specfPL or snapfPL (which is just a + or -) or the new Catalyst test.

If you feel his appetite isn’t good at any time, ask your vet for a prescription for MiraTaz transdermal appetite stimulant. You can get it cheapest at Chewys and it does not have the side effects of any of the pill for of appetite stimulant. It’s really a miracle drug.

@CopernicusDM just checking in to see how Copernicus is doing today. It’s good to see that ketones are down to 2.1 (I might be stalking, er I mean monitoring 😂 his spreadsheet).

I don’t give dosing advice, but have you thought about when you might do a dose increase? I believe @Marje and Gracie had suggested fast-tracking the increases. With ketones in the picture, I would strongly encourage it. (Marje knows her stuff)

Please stalk away! I was trying to get more data. And this platform is a learning curve for me. ;) He needs something. Im confused how he had a pretty good day yesterday followed by a horrible day today. @Marje and Gracie

Hoping Marje has some thoughts on dosing 🥰

I know he had very high bg today. So maybe an increase is in order?

Can you do tight regulation? If so, I would increase him tomorrow to 3.5u bid in the morning as he’s been at 3u six cycles.

If you can do TR, please update your signature and the SS. We can see how he does after four doses. Remember that PM data is also very important as cats tend to go low at night so a PS, +2, and before bed are helpful.

I wonder if since he’s only eating (correct me if I’ve misunderstood) these foods, if he’s not absorbing the nutrients properly.
What I mean is if it’s not cat food (or properly supplemented with a completed with vitamins and nutrients) maybe that’s part of why he’s losing weight?

I know it’s ok to supplement with foods like what you are giving when eating is diminished. But he needs balancing vitamins etc.
Just a thought.
Anyone have thoughts on this?
