Hi All. Can anyone offer some advice?

We still haven't gotten to the bottom of Gryphen's lackluster appetite or vomiting. The vomiting, though, is now similar to what is has been for years; if she doesn't eat for several hours, she vomits clear or bile (or maybe grass, if she ate that). The difference is that usually, if I leave food out, she will eat it if her stomach is upset, but she hasn't been doing that.

I've had every test at the vet that's possible for this area. Some things suggested from this group (and a kidney group on FB) are not available at my vet, and the nearest 'specialist' is at least 7 hours away.

We started transdermal famotadine on Tuesday evening, hoping to stop the AM vomiting (as that's the most common time for her...right around 4am).

Gryphen's BG numbers remain fairly stable, though she does have an occasional spike when her stomach is upset. (As an example, she was a 206 on Monday morning, but she had just been throwing at about 4:30 AM, so I know she wasn't feeling well.) The numbers are higher than I'd like, but according to posts I've found in the OTJ forum, they aren't especially concerning, as they are remaining below 200 on an AlphaTrak2 meter. I'd love to see her in the double digits, but I've only seen that a handful of times on this entire journey so far. She does go lower after she eats.

My vet said that she's not concerned with the BG numbers, but I am. My cat is obviously diabetic, and I want her to be better. I'm afraid that not re-starting the insulin once I can get her back to a regular eating pattern will just set her up for failure, but I'm nervous since my vet said her numbers are mostly in the normal range (per the clinics numbers, normal is 74-159, and she's within those 95% of the time...the AM numbers are typically the bad ones).

She does have a dental scheduled on June 14, as she has some light tartar, but the vet saw nothing concerning upon exam. I'll have them do xrays during the dental, in case there is something happening that can't be seen.

What do you all think? Should I hold off on the insulin? This whole vomiting/not eating fiasco turned in to an unscheduled OTJ trial, and we were not ready...it was too soon!! :arghh::arghh::arghh: Should I maybe wait until after the dental, and then if her numbers stay where they are, talk to the vet about going back on the insulin? Are her numbers high enough to start insulin again? It's all moot if I can't get her back on a regular eating schedule (and if she keeps throwing up in the morning), but there is so much going on with her right now, and we are not getting any answers from the vet/tests...I'm starting to get a bit overwhelmed. Thank you in advance for any input.

Her numbers are t too bad without juice but I think she needs some. I dint think I would wait a whole month. What about going on a drop dose? Let’s see if Bron and Wendy agree. @Bron and Sheba (GA) @Wendy&Neko

Thank you! I think I'm concerned, too, since her numbers stay the same at .25, .1, or none. It's like she's just stuck at these numbers and nothing will make her budge from them!! :banghead::banghead::banghead:

They were lower at .25 but I’d be worried at going back to that with no appetite. you could go back to .10 also.

Have you tried giving slippery elm bark powder on a regular basis to see if that helps with the tummy upsets. You need to keep it 2 hours away from medications.
Can I ask why you swapped to the pet meter?
Would you consider going back to using the human meter? I find is so much easier to help people if they we using the human meter.....we know the normal BGs are 50-120 and they are used for our dosing methods. With the pet meter we say the take action number is 68 but we have no definite numbers to go by.......
I agree with @tiffmaxee. I would start with a drop dose and see how that goes.

They really weren't lower at .25...I think that's the human versus pet meter coming in to play. I was doing one poke two meters for a part of that time (for the vet) and the Alphatrak numbers were actually higher at .25. Such a mess all the way around...

I went back to the pet meter because my vet was asking me for numbers and I couldn't keep poking her multiple times (if she wouldn't bleed), and my vet would not help me with anything if I was trying to give her human meter numbers.

I did, however, use the Contour meter tonight at PMBG, and the number was 134.

Thank you for the input!

I think that’s pretty poor the vet wouldn’t help you unless you used the pet meter. Vets used human meters exclusively until a few years ago and would be familiar with the numbers.
I don’t want to pressure you at all, but if you could switch back for a while, while we help you, that would be very helpful.
Already that 134 with the human meter at PMBG is really helpful. It tells us she is running a bit high.

I'll look in to slippery elm bark! And let's hope I can get her back to eating on the regular...something needs to go our way!

Thanks again!!
I use slippery elm bark powder everyday with Clover, who has intermittent loose poops and lack of appetite. I mix it with the first meal of the day. I find it settles things down and her appetite is better. SEB can be used as a slurry, which I used to use for Sheba, or just as a powder which I use for Clover and just sprinkle it on her food. She doesn’t seem to notice the flavour, if there is one.

I was originally trying to get in to see a colleague of hers, as he advocated for human meters. Then I found out he'll only advocate human meters for dogs, since they can't go in to remission. No one in my area is familiar with human meters for pets (except the Libre, which I find odd since that's a human meter...no different than any other human meter, except it's always on.) :banghead::banghead::banghead:

She endured two meters before...I'm sure she'll be fine. Ha! I'll have to get more human strips, though. :)

The PMBG is her highest number since 4/6/21 (other than the vomiting on Monday). I was actually shocked to see it that high at PM, since her numbers are always lower then. Keeping me on my toes, I guess.

Thanks again!

Is there a particular brand you use?

I use this one which I think I got from iherb but I’m sure most are ok.
I open the capsule and sprinkle the amount that is in the smaller end of the capsule on the food then mix it in.
You might want to start with a smaller amount just to get her used to the taste...if there is a taste.
