? 2/9 Frankie, curve 300’s

I finally did a curve today and it’s essentially been a pretty flat line of 300’s. I’m assuming it’s time to add 0.25 units to the dose starting tomorrow? Do I still need to do the 1:30 am (+6) check when the nadir on the first cycle was so high? I also gave his steroids late so he just got the pred and Budesonide at +3 so it’s unlikely he’ll drop much tonight.
 
Even with SLGS, you are holding doses too long. With SLGS you evaluate the dose every 7 days. I think it's safe to increase by 0.25U tomorrow.

Do you think you can test a little more and follow TR? You would need to get at least 4 tests a day - 2 preshot and 2 mid-cycle. More if it's an active cycle and he goes lower.
I think Frankie would benefit from the faster increases that TR allows. That way he can get to a good dose faster. You can always switch back to SLGS once he sees better numbers.
 
I was really hoping his numbers would be in the 200’s now that the pred is every other day, so I held the insulin the same last week for that reason. The reason I decided to do SLGS was because we are still changing his pred. In 5 days he’ll be getting it just twice a week Then none at all. I’m worried about being too aggressive and then having hypos once the pred is wearing off.
 
I understand your worry. But I am wondering if there is an element of glucose toxicity at play as well. What this means is that your cat's body gets used to being in higher numbers and treats it as the new normal. It's a form of temporary insulin resistance which makes it harder to get the numbers back to the real normal.

With TR you hold doses 5 days (vs 3 days) once you start seeing numbers below 200. So that's just a little faster than SLGS.
