15/3 PMPS 544 help please

Keiki’s levels are still high tonight when I got home! I got a new monitor today so tried that first and the reading was 22.2 (400) but old meter was 30.2(544)

I’ve quickly fed her dinner with lots of water mixed in and gave her 1.5 instead of 1 unit of Glargine

The new meter (lifesmart) tests for ketones so I did a keto test and it was 1.6 - but when I tested her urine (fresh) it was negative for keto ‍♀️

My goodness I don’t know what to do

I guess I’ll keep monitoring every few hours and hope she drops down - actually with her pattern she will be in the blue in the morning what am I doing wrong?
 
Was the old meter a pet meter? Pet meters read higher than human meters. There can also be a 15-20% difference between meters.

Glargine is not dosed based on preshots but on how low it takes the numbers. We usually ask new members to skip insulin when the preshots are less than 150 because they do not have enough data on how their cat behaves on insulin. However, when there are ketones involved, it's better not to skip shots. If you can monitor, you want to shoot any number over (and including) 90 when following SLGS.

The higher numbers you are seeing could be what we refer to as "bouncing".

"Bouncing is simply a natural reaction to what the cat's system perceives as a BG value that is "too low". "Too low" is relative. If a cat is used to BGs in the 200's, 300's, or higher for a long time, then even a BG that drops to 150 can trigger a "bounce". Bouncing can also be triggered if the blood glucose drops too low and/or too fast.The pancreas, then the liver, release glucogon, glycogen and counter-regulatory hormones. The end result is a dumping of "sugar" into the bloodstream to save the cat from going hypoglycemic from a perceived low. The action is often referred to as "liver panic" or "panicky liver". Usually, a bounce will clear kitty's system within 3 days (6 cycles)."

Some cats go high before they break a bounce and it's possible that that's what she's doing today. Like you said yourself, you might just get a lower preshot!

With SLGS you hold a dose for a week and then evaluate the dose based on the nadirs. If you can test enough (at least 2 tests per cycle), you might want to look at following TR. It will allow her to spend more time in better numbers - especially with ketones in the picture.
 
Why does your spreadsheet say you gave 1.5 last night and then there was no further testing and then only one unit this morning?

Sorry i hadn't updated her sheet, I had to go out but I tested when home and she came down alot before bed. This morning just as her pattern says - she was only 112, I gave her the shot anyway (only 1 unit) and have been testing every hour - it is still dropping, her test now is just 58 - however this new meter has already shown it reads lower as yesterday this meter was reading 22.2 (400) and the old meter was 30.2 (544) (I test straight after each other) I didn't bring the old meter with me to work but I may just duck home to get it as this (3.2 / 112) number is a little worrying and its only 3 hours post shot, She just ate another meal so hopefully she was just hungry. I know now don't change the units (the Ketone read last night of 1.7 kinda freaked me out a bit) but its down to 0.5 this morning.
I still don't quite understand this whole thing there is alot of conflicting advice - like the Vet said to start on 2 units, then I realised I was reading the syringe wrong and was giving her 0.5 units instead of 1.5 so now I count down from 5 to get to the 1 as the first line is actually 0. Keiki isn't a big cat she is only 4kgs so not overweight at all (she did get all boney with the UTI but she is putting it back on)
 
Or Al bg is 50-80 or 100. Since you have decided to follow SLGS, reduce the dose to .75 starting tonight. Hold the dose 7 days unless you see a bg under 90.
