? 11/7 Oliver. Is +9=585 OK? New to Lantus, just got out of hospital

My newly diagnosed cat Oliver has been on Vetsulin for past month and got very sick 3 days ago with vomit and diarrhea. Two days in hospital: IV for fluids, antibiotics for high WBC, switched from Vetsulin to Lantus.

I am very worried about his BG's today since I am new to Lantus. AMPS=312, +2=266, +5=355, +7=394, +9=585. Should I do anything or is this OK?

I've been posting on the Health Forum. This is my first post here. Sorry if I didn't post correctly.
 
Hi! Welcome :bighug: please keep your question up so others can take a look but I do know If you gave Lantus this morning, you won't administer another shot until 12 hours after the morning one.

How's Oliver feeling today?

If you are able to, can you edit your signature and replace "Vetsulin" with "Lantus"?

Thanks for noting needed changes in the signature! Ketone urine check daily; has always been negative. Vet said he burns them so I missed them with the urine sticks. I gave him his Lantus (1 unit) this morning, 10 hours ago and will be up to shoot again in in about 2 hours.

Oliver is feeling great! Even asked for his walk outside on his harness.

I'm out of my league here with ketones (no experience) but I'd call the vet like Jeanne said. Have you gotten a ketone test last night or today?

Oliver is most likely bouncing from the greens on 11/5, it can take up to 6 cycles for the bounce to pass. It’s nothing to worry about, the bouncing usually gets less and then disappears as the kitty gets used to being and staying in normal BG numbers again. Keep checking for ketones while he’s running high though.

Lantus is a depot insulin so it works very differently to vetsulin. You’ll need to learn how to think differently but we are all here to help. One thing I can see is you’re feeding half hour before the shot. With lantus you don’t need to load up on food before the shot, you want to test, feed, shoot all within minutes. Your spreadsheet the way that it is is going to be pretty difficult to read for lantus dosing. It’s going to be much easier for us if you have a new page without all the extras, by all means keep the other page going for yourself but when we’re looking at dosing we’re reading the spreadsheet in waves so all the extra information and additional alphatrack readings within the readings makes that very hard.

That's exactly what I accidentaly (in my sleep-deprived stupor) did this morning. I tested, fed, and shot all at the usual 1/2 hour before shooting Vetsulin. So I'll keep doing that.

Do I need to withhold food 2 hours before shooting as with Vetsulin? Oliver hates that and I usually bribe him with catnip at that time. It's one hour to shooting right now and Oliver desperately wants to eat.

I will make a CLEAN spreadsheet. I knew that would help. Thanks. So glad to know he's bouncing.

Hi. I couldn’t get back on the FDMB. You don’t want to feed a half hour before insulin. You want to test, feed, shoot all within 5-10 minutes. It takes about two hours for Lantus to kick in so just a small amount of food is needed at shot time.
 
OK. Thanks. So it's the same as with Vetsulin: no food 2 hours before AMPS and PMPS.

I will test, give a little food, then shoot--all within 5-10 minutes. Is it OK to feed a large meal? He's ravenous. Or should I force him to spread it out?

I made the clean spreadsheet. Now I just have to see if it's in my signature.
 
For some reason the new simplified spreadsheet link won't replace the old one in my signature. I've copied it to the signature twice (saving changes of course) but every time I click on it (and I even gave it a new name) I get the old detailed spreadsheet. :(
 
It looks like you feed meals rather than free feed so it might just be a case of rearranging and/or spreading those a bit so he’s not giving you the hungry sad eyes in the last couple of hours
Great idea! I've been feeding 1 full can every 6 hours. He rings a bell all other times when he wants to eat and I make him exercise for each morsel:). I don't leave food out because I want him to get some exercise (hunger is very motivational.) I will re-think this.
 
I gave the 1 unit Lantus. I just learned a ton of great info. Thanks y'all! I will not be afraid of giving extra food & water since he'll need it while bouncing. Vetsulin has no depot so I'm understanding this is a new ballgame.

Knowing why these high numbers will last several days is so helpful.

Is he overweight?
He is still underweight. He should be around 9 pounds. We got him up from 7 to 8 pounds.
 
I just realized that y'all might think he is overweight because I make him exercise. No, I make him exercise because lack of exercise is typical of diabetic cats and he was one lazy boy! I am not a mean drill sergeant. (I just had to ask my son how to spell "sergeant" and he said, "Mom, you are a mean drill sergeant" :blackeye:.) I cut Oliver a lot of slack if he's not feeling well.
 
Since he’s underweight all the more reason to feed him often and as much as he wants. Exercise is good regardless of weight but if he needs to gain you want to give more food.
 
The reason we like to spread the food is because it’s easier on the pancreas. You don’t need to get a good food buffer in with lantus as compared to vetsulin because it’s a much gentler insulin. Really, the only reason you’re worried about getting any food in at shot time is so that you know Oliver is well and will eat later.
