Hi,

So I had issues with Theodore's dose, we were going up then vet believes he was somogyi, group educated me no such thing and I started raising him to 2.5U, interestingly enough when we raised to 2.5U it looked like progress was being made but now effects of insulin looks like it's slowly down again, any advice? Do I go ahead and raise to 3

Normally with SLGS you would hold the dose for 7 days, but this isn't "normal" circumstances. You are recovering from bad advice. I would go to 3 units. There might even be an argument to go to 3.5, but you did see blue on this dose.

Out of curiosity, why did you shoot 2 units on 8/16 PM? Shooting a reduced dose interrupts the depot, and again in normal circumstances would mean holding the dose a bit longer. But again, you need to get him back on track.

How is Theodore acting/behaving these days? He is probably bouncing from that sweet blue a couple days ago. And with no night time tests (ah hem!) it's hard to say what he's doing then.

Vet asked me to dose according to the below:

<5 mmol/L -> no insulin
6-10 -> 1 unit
11-15 -> 1.5 units
16-20 -> 2 units
21-24 -> 3 units
>24 -> 3.5 units

I feel horrible. My poor Theodore. I gave him 3 units tonight. Just have to update the spreadsheet :)

Theodore has been OK, I am not sure, during the day he seems like a slug, he is eating and drinking well. I think thirst is still excessive along with peeing. I would really like to see him gaining weight. He's at 14.6 lbs, I would like him to gain weight. His lack of back muscle really saddens me.

How old is Theo? Though not always telling, some cats slow down with age. I also meant to ask, any ketones tests recently?

Sounds like the vet isn't that familiar with Lantus dosing - not uncommon.

I found him in 2018. The vet said he's probably around 4 years old but they didn't really know. He was missing teeth, had broken teeth, FIV+ but he was the friendliest black kitty. I had him cleaned up, neutered, etc. The problems started when we did a full dental extraction (almost full), megacolon started. I will say that we do have that well under control now with him taking extra large poops that bring us the biggest smiles.

He is definitely a lazy boy but jumped in bed twice last night as it was my husband's first night back from his work trip. He also will slap a string around with his front paws but even in 2021 he would chase it more.

Yea, I was looking back at videos in December and even then he was playful. Given he’s had diabetes since probably March at least, do you think there’s irreversible damage? His blood work end of June was really good but I worry with his lack of playfulness

He should get more active once he starts spending more time in good blood sugar numbers. It took Neko a long time to get into better numbers, much longer than Theo has been in higher numbers so far.

Thank you for re-assuring me. I am finding this is really testing my patience. I just want my little buddy to feel better. :( Just got another reading at +2 now of 31.2
