Hello again! We will be starting Evie on her Lantus soon just picked it up from the pharmacy. And her new needles .
I’ve had a hard time with my vet trying to first convince him to even write me the prescription for Lantus; and second what her starting dose should be. I know the lovely ladies here agreed 0.5 units seems good for Evie based on her spreadsheet and previous numbers on Prozinc ,
I think I agree with this!

But my vet went by the information in the VIN Veterinary Drug Handbook and based on her weight weight of 4.35kg and dosing at the low end of 0.25 unit/kg, he recommended one unit. He said that dosing is based on weight on not Nadir? And he also said to start every 24 hours at first to play it safe?

I’m thinking this is ridiculous .., please confirm and tell me your thoughts !!
 
Looking at your SS now, I would start at 0.75 unit. You have been giving 0.75 U for 8 days and it has not dropped Evie into greens.
Also please give the dose every 12 hours as you have been doing. And the dose is based on the nadir when swapping to another insulin and looking at how the other insulin effected the BGs.
 
Welcome to LLB . I'm a former Prozinc user with a dose of 3units and when we switched to Lantus we started at 3units. My Vet wanted me to start back at 1. I nodded my head and smiled and thanked him for the rx. When we got home, we started my Cleo on 3 just as this board recommended ;).

Have you picked a dosing method? There is a lot of information in the stickies at the top of this sub forum.

Thank you! yes she hasn’t dropped much in the last week . I’m a wimp and not home mid day to monitor so I hate increases ! Since we are switching anyways to Lantus soon I do believe this is good advise .
 
Did you have better luck with Lantus?
Absolutely yes!!! The depot nature of this insulin (eventually) stopped the big swings in BGs and has more predictability than when on Prozinc.
It took me some time to wrap my head around the 'depot' concept and not have anxiety when getting greens. We're both (cat and human) much better :)
 
Hello and welcome to LLB forum. Your vet is right on quoting what the starting dose would be for Lantus - but only if brand new to insulin. With a track record with another insulin, we take that dose into consideration.

Can you make the switch on a day when you can monitor more closely? We see the odd cat that has a rather more dramatic response to a new insulin.

Yes I will do it on a day when I’m home with her all day! This is why I was leaning towards a starting dose of 0.5 just to be safe?? Or do you agree 0.75 is good for her . Prozinc has been very unpredictable and I’ve been assessing and giving new doses every 7-10 days I feel. One week 0.5 works almost too much, other weeks 0.75 hardly works at all.
