Hello FDMB,

A few things have been going on that we would like to get some support with.

First, we recently switched to the TR dosing method. We think TR will be a better fit for Shelly since she often seems stable on a dose and a reduction under 90 makes her numbers fluctuate a lot. Though we are conflicted about how we will feel shooting at lower numbers, we are hopeful this method will help us better regulate her and we know we can always change back if it's not a good fit.

That being said, we are currently dealing with what we think are IBD symptoms. We have been struggling to find a food she will eat. Our options are already somewhat limited because of her preexisting conditions – the IBD and the formation of bladder crystals.

As some of you know, she had been on Royal Canin PD (16% MC) for her IBD and crystal blocking support, but when she had an IBD flareup recently while on that food, her vet posited that the budesonide could be thinning her urine enough to not worry about the crystals and suggested we try Rayne nutrition. Long story short, that was a miss, as well as a number of novel proteins we tried off the shelf. We are currently having some sucess with a rehydrated freeze dried pork, fingers crossed. There is also a Lotus pork in the mix. We also added a probiotic to her regimen and we are hopeful that is supporting her digestion.

The budesonide is definitely not a perfect fit in terms of regulating her IBD, but we don't want to return to the prednisolone if we can avoid it (for the obvious reason that it caused the diabetes).

We are holding her current dose of 0.5 until we get her food stabilized. We would like to try 0.75 when she is eating more consistently. Does that sound like a good plan?

Also, for those who offer meals throughout the day, have you found a good system for that? She doesn't eat a lot of food at once so it makes it challenging. Right now some food is being left out, but as we incorpoate the freeze dried we want that to be more of a 'eat it now or it needs to go away' type system and get her used to that. This morning she had 1/4 c. rehydrated freeze dried that she ate in two sittings 30 minutes apart. Then 1/2 small can of the Lotus pork that she ate most of for a snack. With the IBD she is more of a grazer but with the DM we want her to have more defined meals. How do we reconcile that?

Grazing is good for fd. My boy had chronic pancreatitis and needed small meals more often and it worked out really well. The first few times or more you need to deal with lower numbers post asking for assistance. Someone will guide you through the cycle. If you shoot green and anyone tells you to shoot we have a system where the advisor will stay with you until you are in high enough numbers. Good luck with TR.
 
For the "eat or it will go away" system, have you tried a rotating automated feeder for wet food? Grazing is also fine for diabetics.

Identity is another brand that has a pork option.

You can increase her to 0.75 units any time. Have you cross checked any of the Libre greens with the Relion? Sometimes Libre tests lower than it really is below 100.

Don't forget to update your signature to say TR now. Good luck with TR as the dosing method.

We have thought about getting an automatic feeder…

We have not cross checked much. Honestly I have been struggling to get enough blood for a test. I have read about it and watched the videos and I can’t seem to get a large enough droplet most of the time 😭 I plan to try the Vaseline method next to see if that will help form the droplet.

It can take some time for the ear to "learn" to bleed. On a darker cat, I understand that using Vaseline helps the droplet to bead up better. Are you warming the ear with a rice sock? Also, are you ensuring that the bevel is up on the lancet?? I'm not sure if you are free-handing or using a lancet device. I free hand because the click of the lancet device scares Jude.

Yes, I am warming the ear with a DIY mini water bottle.

I’m using a lancet. ReliOn brand. I haven’t tried free hand yet.

I’m getting a droplet about 75% of the time but when I try to milk it out I don’t get enough blood and then I get an error reading on the meter.
 
What size lancet are you using? Best to start with a larger one, size 26 or 28. Often the ones that come with the meter are higher numbers/smaller lancets.

That’s a good question. How do you know what size the lancet is? I just looked up the one we’re using and it just says “ultra thin” but I don’t see any numbers associated with the lancets.

Wendy, I wanted to circle back and let you know we ordered the 26 gauge lancets and I got a reading on my first try! It was a very nice droplet that easily filled the test strip. Thank you so much. I was feeling very deflated. Today I was able to cross check a Libre number – 53 on the Libre and 58 on the ReliOn. So nice to be able to do that!
