Hello! We have been doing really well since Dec 2024! I was suspecting that Baby wasn’t doing well. She was eating more and she was hiding in my room which raised a flag for me. I checked her numbers and she was high! I had been checking her 2 times a month and she was doing well for the last few years. She is running in the mid 300’s- to high 400’s. I have bee on Fancy feast classic pate. Please advise where to start. I am dreading the vet! I don’t have much trust in them. You helped me save her and you all taught me how to take care and monitor her to the point that she was OTJ.
Any suggestions are welcomed and needed.

Since I have been gone I have rescued 5 other cats. I initially suspected that Baby got into the other cat food. Typically if that happened I was able to get her stabilized within a few days. She is not rebounding this time. I do have an insulin pen but I am hoping to hear from one of you before making the next decision on what to do. I keep testing her every 30 min to 1 hour when I am at home.

Because she has a history of DKA, I would definitely test for ketones until you figure out your next steps.

You can test the urine for ketones using ketostix (which you can get from a pharmacy or online). Or you can get a blood ketone meter and check for ketones on the blood.

Thank you for the response. I appreciate you. I used to have a blood ketone meter. I will order another one. Either way I need to do something to help her get stabilized. Back when she had ketones the vet was ridiculous and told me I needed to hospitalize her and it was going to cost me 10 grand etc..and it wasn’t hopeful for baby. I took her home and we recovered. The ketones dropped when I stabilized her blood glucose levels. I have been reading all night about starting her back on the insulin. I am checking her glucose levels now.😔😔

I did order the ketone meter. It will be here today. I am leaning towards starting her on the SLGS dosing method with the Lantus insulin at .5 dose. I have not done that at this time but I am thinking out loud. 🙏I don’t want to be back where we were a few years ago. Baby is currently eating and drinking and using the litter box. I can tell by her demeanor she isn’t not feeling great but she is moving and responding to me pretty well. I am definitely catching this fast and early. Thank you for taking the time to respond. Well done getting Ila back on insulin. Lantus is a good insulin.
Are you still using the solostar pen?
I would recommend you change over to a syringe. You can still use the same cartridge you have in the solostar pen. You will be able to give smaller doses later on.

Also I can see you are doing the SLGS dosing Method.
I would recommend you read the tight regulation method and swap over to that. You are testing enough to do that and are feeding only wet food.
At the moment you are dropping below 90 and if you are following SLGS you would have to reduce the dose. With a relapsed kitty it is better to be more aggressive with the dosing if you can. You will have a better chance of remission again.
With the TR you would reduce the dose when it drops under 50 on a human meter.

Hi! thank you soo soo much for this guidance. I am using a syringe and extracting the insulin out of the pen. I will happily change over to the tight regulation. I will read about it right now! I did get the Weruva brand cat food today. She loves it thankfully. I am giving her fluids Subq 50-100 units once a day. I have to enter her labs so you can see where she is overall. I have not received the urinalysis results yet.
Baby is hanging in there.

Hi @Bron and Sheba (GA) I took your advice and switched over to tight regulation. Just to make sure I understand correctly since Baby jumped up above 200 I increased by .25. I know she had a number slightly above 300. I was nervous to jump to .5 increase. She had a nice response later in the morning on the 16th. Since she is creeping back up this evening would that mean that I increase again?
I have read the tight regulation explanation over and over. I just want to make sure I am following it correctly.

Hi Laurie
I’m not sure why you decreased to 0.25 units on 15/9. Baby did not drop under 50.
So 0.5 units is the correct dose at the moment.
While Baby is giving you green numbers you do not increase the dose. Lantus dosing is based on how LOW the dose is taking the kitty.
Those higher numbers are from bouncing.
Dies this make sense?
