11/16 Mo Dosing Question

My cat Mo was diagnosed 2/13/2025 with Diabetes. The vet had prescribed him Novolin N which we have use since February. We just got a prescription for Lantus because we heard it was better and the Novolin didn't seem to be working for him. We were able to get the Lantus Pens and will be using the u-100 needles to dose it out. His most recent dose on Novolin was 2.5 units 2x a day. When the vet prescribed Lantus he said to give him 1.25 units 2x a day. We started this morning. Is that too low of a starting dose? I know Lantus is a depot insulin and not fast acting (harsh) like Novolin. Should I just try the dose for a week and see how his numbers look? He always seems to be in the black (High) numbers a lot.
 
We usually suggest slightly less than the dose of the current insulin. I’d start with 2.25. It will take 5-7 days for the depot to build. Let’s get another opinion. @Wendy&Neko

Typically we consider how the previous insulin/dose was doing when suggesting the dose for Lantus. 2.5 units was getting him down to low 100's. Looks like you have some days where you can't get tests during the day, I presume due to work. 2.25 might be a safer option, just in case he has a stronger reaction to Lantus.

One thing you will have to learn with Lantus is giving insulin with much lower preshots. Lantus does great at lasting the cycle, so you will begin to see those lower preshots as you get closer to a good dose.

Are you home testing for ketones now? With those high numbers, ketones testing is like insurance.

Thanks so much for your reply. I will start him on 2.25 units tonight. No I have not tested for ketones. I will get some strips on my next shopping trip!
 
I was nervous about the ketone testing strips, but it's real easy.
I just took the lids off of all my litter boxes and waited close by. Once they start peeing, it's very hard to stop. It's inch closer and not look at him when he got in. And as soon as he started, leaned in to put it behind him. They look at you funny the first time or two.😅

I do the same thing with the ketostix, just wait until my girl is done peeing and stick the strip in the puddle quickly before pee sinks into the litter. Works really well.
The hardest part is being there when they pee. But I kind of know her schedule (2 hours after her meal) so I watch her on the pet cam and as soon as I see her going to the box I get in the room and get the strip ready.
She's used to me jumping in calmly to get a pee test :joyful:
