Hi there. Wendy is out of town right now. If Marvin drops below your reduction point of 70, you would want to reduce his dose by about ten percent. So if his dose is 12.5 that would be a reduction of 1.25 units, which would bring you down to 11.25 units. If you want to, you could just reduce it back to 11 units to make drawing the dose easier.

One more question, do you have an opinion on whether it is better to give 1ml of Cabergoline EOD or 0.5ml everyday?

Since the typical starting dose for Cabergoline is 10 mcg per kg. of body weight, he should be getting about 63.5 mcg. He’s approximately 6.35 kg. So it sounds like they are telling you to administer .5 mL of your Cabergoline solution every other day, right?
 
I started my boy on daily dosing of Cabergoline. Then I experimented with every other day and eventually went back to daily. We have had people who started out doing it EOD and then increased to daily.
 
Oh. I see they’re telling you to give a whole mL every other day. That would be a high starting dose of approximately 20 mcg per kg body weight. If it were my cat, I would do the .5 mL daily in order to try to minimize GI side effects.
 
Kwyn, did you not reduce the dose when Marvin dropped below 50 a couple of days ago? I know he’s a long term diabetic, but just watch him carefully because you are starting Cabergoline. I know it doesn’t usually start to work immediately, but just be vigilant. He just re-started insulin in October 2024 and he’s also new to these lower green numbers.

Yes the prescription says 1ml eod but I will do 0.5ml everyday.
I didn’t reduce his dose when he dropped below 50 as I was following the instructions I was given because he is a long term diabetic. We are back up in the 300s in today’s pm cycle. Fortunately I work from home so I can keep an eye on him and test often.
I asked the pharmacist if it was ok to give him the Cabergoline at the same time as his insulin but she didn’t know as she is not familiar with diabetic cats. Would that be ok or should I space it out?

I gave my Cabergoline first thing in the morning with the AMPS meal. I don’t recall anyone else here waiting to do it at a different time.

I would not follow that long-term diabetic guideline on reductions in Marvin’s case as he is an acrocat with IAA and is also starting Cabergoline. Extra caution is warranted as there are many factors in play (growth hormone, antibodies, Cabergoline). That’s why Wendy suggested a more conservative reduction point of 70 instead of the usual 50. She commented on your previous thread also - to ignore the long-term diabetic guideline in Marvin’s case.
