? 1/18 Chispa, 391 AMPS, 196 +2.75 - next steps?

Chispa continued to eat small amounts last night -- not much, but something. The vet who saw her yesterday called this morning and said her labs were pretty normal except for potassium, which was very low at 2.7 (normal is 3.4 - 5.6). I've long been giving her potassium supplements mixed with her food ... which she hasn't been eating ... so that made sense. Vet recommended I take her to ER for supplementation, but when I questioned, she confirmed that it goes in and out of the system pretty quickly and would probably rise if she would take her meds at home. So far I've been able to get 2/3 of her daily dose into her mixed with Churus and hopefully will continue throughout the day. That's the good news.

My more immediate concern is getting the rest of her meds into her. She's not accepting them in treats, and I'm not confident about pilling her (never had to do). However, the vet said one effect of low potassium is inappetence, so maybe she'll eat the treats if her level goes up? I think I will give it another few hours before rushing off to the ER. She's not in any obvious distress, and her face doesn't look nauseated today. I don't want to overreact (and pay an arm and a leg, AND put her in a stressful position) if she can improve at home. Is that reasonable?

The vet we saw yesterday was quite young, not that that's a bad thing, but I have to wonder if she was being overly cautious with the ER suggestion. She said raising the potassium level had to be done "very, very carefully," but even if I can get Chispa's level to double in the next 24 hours, she'll still be within the normal range. The vet didn't want to prescribe Mirataz or Cerenia because of possible interactions with Chispa's other meds, but I looked at the Mirataz website this morning and saw that the only interactions are with MAOIs, which Chispa is not taking. The Cerenia website says it should be used "WITH CAUTION" in pets with known heart disease, which Chispa has, but very early stage, not even enough to require meds. I guess that's why I'm taking her advice with a grain of salt.

Update: She vomited a small amount of clear foam while I was typing this. She's been drinking her normal amount of water, peeing regularly, and was pooping like a champ until this morning, when she produced a very soft stool, possibly from eating unfamiliar food.

Should I continue to try to get food into her at home, or just take her to the ER?
One benefit I just thought of would be that they could sedate her and check her teeth to see if that's the problem.

UPDATE:
She had diarrhea about 20 minutes ago. Just now she ate a Churu mixed with potassium supplement -- and then pawed at the side of her mouth.
So I think it's a dental problem.
I'm going to call the ER place and ask them what to do. I doubt they'd be prepared to pull teeth today, even if they could get a look inside her mouth -- in fact, they'd have to do a ProBNP test first to know if she could tolerate anesthesia.

I called the ER. They said they couldn't do anything about her teeth on the weekend, so I could either bring her in or wait and continue to monitor. But she hasn't eaten much except Churus for going on 4 days now, and that's not exactly a complete and balanced diet, so I am going to bring her in and let them try to get her potassium and diarrhea and appetite and whatever else she needs under control. I hate to put her through the ordeal, but it seems like the safest and most helpful thing.

I’m so sorry Chispa hasn’t been eating well or feeling well.
That sounds like a safe call to take her to the ER and have them take a look. Maybe they would want to give her fluids or something. But they would be in a better position to assess her.
Sending lots of positive vines that everything goes well and you’ll be out of there quickly and Chispa will get a little help to be feeling back to her normal self again.

Hi Karen, so sorry you're going through this. If it were me, I'd take her in too. Low potassium could be contributing to why she isn't eating and overall not feeling well. At the very least, they should be able to get her electrolytes balanced. Keep us posted. :bighug::bighug:

It always feels like such a betrayal to let them take her back there without me, but that’s how they do it and I understand. I’ll update as possible — please keep the good thoughts coming.
 
I hope the ER can find some answers for you. :bighug::bighug::bighug: I'm glad you made the decision to take her in. You can't treat if you don't know what it is.

Before you mentioned the "paws at mouth" part, everything else you said reminded me when my IBD/SCL kitty got ileus, which required an ultrasound to diagnose. Inappetance and low potassium, though not that low, and a bit of diarrhea. Treatment was fluids as Cecile mentioned, and potassium, and Mirataz to get her to eat. The potassium I got was a liquid (Thera-K) so much easier to make sure she got the right amount. Lack of food also contributed to lower potassium. Side note, Neko, who had much worse heart issues, got Cerenia. Mirataz wasn't around then.

Time to go outside and find a truckload of vines for Chispa.

Hey everyone, thanks so much for all your good wishes. I was on my way home from leaving her at the hospital, when they called me to say that they had found a very large mass in her intestine and another near her adrenal gland. The only options would be surgery to remove it, followed by chemotherapy, and obviously that would be so unfair to do when she has already been through so much and beaten, the odds for me so many times. So I am on my way to say goodbye and give her a peaceful passing. I thought I should tell you all first because I know you love her too. If you could, please say prayers and send good wishes so that she’ll have an easy flight on a soft landing when she earns her wings. I know her best, buddy Chulo will be there waiting to greet her, along with all the other sweet kitties who have gone ahead. Thank you all so much for being there and for being the best support a kitty and a bean could ever hope for. Love, Karen and Chispa

Bless her heart. The both of you were trying so hard, but she has so many other serious issues going on that are working against her that you didn't know about. I'm so glad you made the decision to go on into the ER vet to get her checked out. Giving her a peaceful passing is the best decision--the best gift you can give her. Please update us when you are able after you get home.
 
Oh ((((Karen))))) I’m so sorry. Not sweet Chispa. I know that’s not the outcome you expected today. It was a good decision to have her examined. You didn’t want her to suffer and you made the kindest, yet most difficult decision. I know your heart is broken.

I’m just so sad for you to lose your sweet girl. She’s such a strong and loving girl.

I know what you decided is an act of love, it is so, so hard but I am sure they thank us for letting them go in peace and with our arms.

Jesus Christ ! Sorry I just had to say that Karen. I am so so sorry and crying while I type this. You have taken so good care of Chispa, the best mama she could ask for. To many kitties have passed recently that it makes me sick to my stomach. I know Chulo will be waiting for her with lots of crinkly paper for her since she loved to lay on it. Thank you all so, so very much for your kind and loving messages. I was so exhausted last night that it was all I could do to light some candles (the battery operated kind) near her picture and fall asleep hugging her favorite soft blanket. I felt that she was with me and still is, and I don't think it's just my imagination. She was the best and most loyal friend anyone could hope for.

I'll gather my courage and post a GA later with pictures and more details, but for now, I just want you all to know how much it means to read your messages and hear over and over that I've done the right thing in letting her go. I talked it over with two excellent vets (because there was a shift change) and a very kind tech, and they all said the same thing: she would go downhill very fast, maybe a few days at most, and this is the best and kindest thing to do. She fell asleep very peacefully in my arms, and I stayed with her for quite a long time -- I don't know how long, but it was very late when I got home.

I've shed many tears and there will be many more to come, but more than anything, I'm grateful -- that she chose me to spend her long and happy life with, and that she didn't suffer for too long.

Wendy ... how can I ever thank you for all the support you've given us over these past few years? You are a hero to all kitties, and I know Chispa would be the first to say so. Angela ... you were one of my first friends here. I remember how difficult, how frustrating, how impossible it all felt at first -- and then I would check in here and see your posts about Cleo and realize how lucky I was to have Chispa's cooperation!! It's inspiring to look at your spreadsheet now and see how far you two have come.
