Hi! We kinda need some advice now. After two days of relatively flat ~300. His remained 350+ today. And now he is at 486 PMPS.

I have a few questions:

1. Do I adjust him insulin dosage now by 0.5? Or should I still wait until tmr morning?
2. If this is not a bounce, I am really confused as how is he so high now on higher dose of insulin and LC food? Could this be something else?

Update: ended up shooting him 5.5 (a 0.25u increase) given that he did have 265~280 in the last three days and it has been 6 cycles since PMPS 10/10.
 
It's good that you increased. Dose changes between 5U and 10U are in multiples of 0.5U and not 0.25U, so you can take him up to 5.75U tomorrow morning.

Do you have any thoughts on why are we seeing consistently high glucose with increased dosage and LC food? Do you think perhaps the Glycobalance (Diabetes Rx food with 14% carb) was indeed helping him?
 
Do you have data from before? I am curious as to why you say he was doing better before the switch to LC.

I don't really have anything on hands anymore but they were mostly similar to the Free Libre results in early August on his sheet. I guess since he had green / blue / green at 4.5 and HC food, I was expecting significant improvement with LC and 5.25:banghead:

And it is more of a gut feeling I guess. For about 2~3 months he didn't really vomit (maybe once per week or per two weeks). His fur (especially on his back toward his tail) was smoother and way less oily.

I don't think his numbers are worse now than they were in August. It's normal for a cat's insulin requirements to go up and down. I see your point though. Normally, there should have been a significant improvement from a change to a low carb diet. Is there a chance that he is getting into HC food without your knowledge? When were his teeth checked last? Any kind of infection/inflammation can affect BG and dental issues are one of the reasons that aren't always obvious.

+2 at 366, do you think he needs to be tested at +4 and +6?

- That's exactly what confuses me the most. While I understand how his insulin dosage may need adjustment from time to time, I was expecting a better curve / lower insulin dosage needed after switching to LC food.

- It is highly unlikely. He has no access to HC food and I keep a pretty tight control on that. Especially in the last a few days, I have been basically with him 24-7 so I cannot imagine how was he getting to HC food for other cats. In addition, as I mentioned, he has been pooping way less and less frequent since switching to grain-free LC food, and this has not changed in the last a few days.

- I will ask the doctor about this. About a month ago his doctor did mention that he needs a teeth clean but he is not comfortable doing it while he is not well regulated.

- His +4 came in at 319. It doesn't seem like his BG is diving. Set auto feeder at 6, 8, 10. Will test AMPS.
