911 - DKA, HYPOTHERMIA, BG >600

Left for a vacation for 4 days. Left mom in charge of cat and shots. Came home early bc she said he was vomiting and didn’t eat. When we found him he was soaking wet. He didn’t smell like urine but could’ve been. He also could’ve been living in the bathtub drinking goads of water. He felt extremely cold to touch. Rectal temp was around 90 degrees F. Stuck him on a heating pad and wrapped in warm blankets and a few mins later it went up to 92.5 F. Hubby tested blood sugar and it was over 400 and he just gave him a SHOT! I think he is suffering a hypo episode and maybe bounced up. Not sure what else could be going on. After our regular vet ER would not take us to being maxed out we found another vet and hubby is rushing over there with him now. I’m not sure if he’s going to make it. Help. Advice. Encouragement. Please. Ozzy is a chronic long term diabetic cat for almost 6 yrs now. He’s had good regulation on one unit 2x/day. I know he gets stressed when we leave and had my mom sit with him. She has taken good care of him in the past. He was so lethargic he could not even stand. :(
 
Im very glad you have taken him to the vet.
Please get them to check for ketones. He may have DKA if he has been vomiting and not eating.
 
He’s at the vet now. They did a sugar check at its over 600. Hypothermic. He’s back now getting treatment. Waiting for more info. Wondering if he’s in DKA also. He is prone to it as he had it 6 yrs ago when he was first diagnosed. No clue how his sugar is so high when he’s been getting his two shots of insulin daily.

Is there a chance Ozzy wasn't eating for your mother? Can't really help too much without a current spreadsheet, but sending loads of healing vines your way. Glad to hear he's at the vet.
 
He’s at the vet now. They did a sugar check at its over 600. Hypothermic. He’s back now getting treatment. Waiting for more info. Wondering if he’s in DKA also. Check to see they have done a ketone test....very important!. If it is DKA he is in the best place and hopefully they have got it early

So happy to see you here!! It’s def possible. She said he didn’t eat last night or this morning but still have him a shot. He vomited 2xs. I was thinking he may have had a hypo episode but vet just said his sugar was over 600. He was over 400 w relion monitor at home. Hubby gave him a shot then.
Now I’m thinking DKA possibly. He has that before when first diagnosed.
He gets incredibly stressed when we leave.
Sorry about SS. We don’t keep it updated anymore. He’s going on 7 years as chronic long term diabetic and always has been well maintained on his current dose. I know things can change so shame on us for that but we chose quality of life for him rather than daily poking. And again, after almost 7 years he’s been doing so good. Perfect labs always besides diabetes.

He has had 2 boughts of pancreatitis before so not sure if that’s what’s going on. I believe he had a fever then not hypothermia.

Probably the difference between a pet meter and a human Relion meter. Might actually be close to the same if tested both times on the Relion.
I agree. I know they can vary a lot a high BG. We didn’t have time to do another sugar test. Wrapped him in warm blankets and had hubby rush him. He was so sick. My heart is breaking. I didn’t even get to kiss him good bye. I’m sick as all hell with stomach bug so I couldn’t take him.
 
Update: Vet came back to talk to my husband and said Ozzy had ketones in his urine and very low potassium. He’s in DKA. Will require a 3-day hospitalization. Last DKA bill (well first which is when we first found out he was diabetic) was over $5k. Praying it’s not that high again this time.

I’m kinda confused. He gets two insulin shots a day daily and my mom said she only missed one shot today. He can’t go into DKA after only 1 missed shot. Something else going on. Maybe his insulin needs increased recently and we were not aware.
 
A factor in DKA is inflammation/infection, pancreatitis means inflammation. Add in not enough insulin and not eating enough and the risk factors are there. I'm afraid more testing is in the immediate future. Paws crossed everywhere that he gets through this. :bighug::bighug::bighug:

Thanks. Yes agreed. Wish I was there instead of hubby. He doesn’t know all the right questions to ask. Waiting for them to come back and hoping can do a call w Dr so I can ask my questions. I’m sick as a dog right now so he went instead.

DO you have to make the decision right now? Can you see how he goes for 24 hours?
Just talked at length with the vet. She told me his condition is really poor. She can’t even guarantee he will make it through the treatment and could come home in 3 days and relapse. She said his kidneys are affected and that he likely has more underlying medical conditions. She couldn’t believe he was a 16 yr old cat with DM for almost 7 years. Said most cats don’t make it past 2-3 years with DM. This is a testament to how well loved and cared for Ozzy is and our dedication to him. I’m not sure what 24 hours will do beside makes him more sick without treatment. She said he looks so bad. She told us putting him down was the kindest option at this point.

My heart is breaking.
