Good morning,

Here is Eddie yesterday:

I had an appointment with the radiation oncologist from VCA Victoria regarding Eddie, his post-SRT status and his Prednisolone dose, which is currently 2.5mg/2x daily.

I told her that I was not going to subject Eddie to any more imaging since he would not be a candidate for any more interventions even if there was regrowth. She agreed.

As for the Prednisolone, she strongly recommended keeping him on 2.5/2x daily for one more month and then if he is not showing any neurological concerns to reduce the dose to 2.5mg/1x daily. Her immediate concern is what she called "early delayed reaction" (EDR) to radiation, which she says can happen in the 4-5 month period following SRT. Eddie is now today 4 months post-SRT.

She emphasized that endocrine response to SRT is varied to the extent that it doesn't necessarily result in insulin reduction or for that matter cessation. She asked that I contact her in one month and report on Eddie: pretty much the 5Rs. She was pleased that he was not displaying any neurological signs and that he was eatiing, drinking, kittyboxing in a good way.

I will, therefore, keep Eddie on the current Prednisolone dose for another month and check in at that time.

At this point, I'm thinking we may, however, have to go from 13.75u to 14u.

What a bummer! In all the cats here that have had SRT, I've never seen anyone of them have to go onto pred for more than a week or so, or even heard the term "early delayed reaction" to radiation. I know Eddie's case is unique, but this does seem excessive. I did some searching, and found this page on the NSCU website about SRT after effects. It says steroids for six weeks.

At least 14 units is much easier to measure! He's been on the current dose long enough to show it's not enough.

Thanks for link, Wendy. I see they mention EDR to radiation: "Early-delayed side effects: up to 40% of pets can have worsening of their original symptoms in the first 1-5 months after RT (due to irritation of the normal brain tissue surrounding the dying tumor). Symptoms are usually mild and resolve in a week or two. Medications might be used to support your pet through this period. Rarely (fewer than 5% of cases), these side effects can be more severe, and could even result in death." Phew.
It's a drag to keep him on the Pred for another month but she said she's concerned for his safety. I think this also might be a ECID situation and she deems this appropriate because Eddie...
Going to 14 tonight...

:bighug::bighug::bighug::bighug: I know you have to be exhausted with all this! I am happy to hear that Eddie is still doing all the kitty things:cat: hold onto that and let's all send out prayers and keep positive! I'm a firm believer in positive out is positive back! (Even though I do sometimes get to the end of my rope...but just braid more rope and keep going)

I hear you about rope-braiding! :cat::D:cat: For the most part, I can do positive. It's only when I really am tired that exhaustion becomes existential as well as physical. But I really appreciated your post on that topic!! :bighug::bighug::bighug:

Yes it does become physical and I too succumb to the feelings that it brings...like last night. It all becomes too much:bighug::bighug:

Was at work all day and got home late: realized I had forgotten to pick up the test strips!! . Have enough for tonight and tomorrow am for both Eddie & Blue. Sheesh!!!
