1/16 Cleo AMPS 280 +5 145 PMPS 223

She was SCREAMING at me to eat @+5. Tested and got the 145 so I guess she was hungry.
Got a call from my Vet with the blood test results from Saturday's visit. I've added them to the labs sheet in my SS. He did say that her SDMA came back at 14 and he said that with that, he anticipates she'll be a CKD cat in 12-24 months. He wants to see her in 6 months for another full blood panel. For some reason, the lab didn't run the thyroid tests - so I got my money back for that!
@Suzanne & Darcy - when you get the chance, would you mind taking a look at the results run by ANTECH labs?

I am looking at them now but - right off the bat - what is up with that Fructosamine number? 555? Seriously? Is that a typo or a lab error? Her numbers are quite good so how could it be that high?

Question: has she been eating and drinking all right? Seems like she could be a little dehydrated? With some of those numbers being elevated specifically the RBC, HGB, and HCT as well as the Albumin, I question whether she may be mildly dehydrated. Are you adding water to her food?

And as you already know, the Phosphorus is too high. What is she eating? In order to lower her Phosphorus and prolong the life of her kidneys (that’s the theory anyway) you should consider switching her to the Weruva BFF PLAY pates which are also low carb. This has really kept my cats’ phosphorous levels under control. There’s have gone down a lot since switching that food. There are others to choose from, but if you can find a flavor she likes in this food then I would highly recommend it.

Her urine pH is too high. That does potentially put her at risk for urinary tract problems like FLUTD. It also creates an environment for bacterial overgrowth in the urinary tract. Doesn’t she have that already? Higher urine pH can also promote urinary struvite crystals. I would ask my vet about ways to acidify her urine.

@Suzanne & Darcy YOU ARE A SAINT for responding so quickly :bighug:

I know this is probably premature, but knowing the SDMA is 14 (right on the edge) and the phosphorus creeping up slowly over the last year, I thought it best to implement whatever changes I can to slow the progression.

Funny - I told the Vet that her fructosamine would come back high. I'm using the FSL which tends to read lower and even though she's been seeing better numbers overall, the bounces are awfully high, especially the last three weeks and are taking the full 3days/6cycles to clear.

I question whether she may be mildly dehydrated. Are you adding water to her food?
I do add a tsp to her feedings/snacks. She rarely touches her cup of water. She also won't drink from a fountain.
She seemed a little off last week - sneezing a bit more than usual but she's perked up as of last night and today. I'll start adding more water to her meals and snacks. Plus her PH is way high and she has FIC. Will added water help with this?

She's getting a mix of foods -
Ziwi Peak pates (but they've been out of stock)
Weruva Truluxe - Steak Frites, Glam & Punk and On the Cat Wok; Weruva Cats in the Kitchen Lamburgerini
KOHA LID pates https://drive.google.com/file/d/1oyedGjzZ0A4pyKGKLud4GXXJ95OFWq9e/view?usp=sharing (link to nutritional info)
KOHA Duck Stew https://drive.google.com/file/d/15pNVb5wBJNY_naJkdYciEYX-12PMIEyi/view?usp=sharing
And KOHA shreds as her MC https://drive.google.com/file/d/1OhxtabZsKVadGadSP_gmQdojYIfKWtA3/view?usp=sharing

She is notorious for suffering from Food Boredom and I usually have to rotate flavors & textures. I'll do a deeper dive into lower end phosphorus foods. I have the link to the list Andrew's list of CKD foods and the one that is also diabetic and ckd.

She's not a fan of chicken pate/shred/chunk unless I cook it .

What should I be looking for in terms of Phosphorus mg/100kcal and on a DMB?
I have been trying to keep her on food that represents itself at phosphorus at under 300mg/kcal but I have no clue what that would be in DMB.
Any guidance is greatly appreciated.

My local mom&pop pet store gave me a sample of weruva's WX food, but I haven't tried it on her yet. Maybe I should supplement the WX food as her after nadir snacks since its low carb and low phosphorus?

As for SDMA, don’t get too worried about it yet. I have had cats with an SDMA of 14 and they stayed there for years. I also have had cats with an SDMA of 14 one time and then a lower number on the next blood panel. Her creatinine has increased a bit since her last bloodwork was done. Because pf this and the phosphorus, I really would start doing the preemptive things like trying to lower her phosphorus and trying to maintain hydration. I think it is reasonable to do the 6 month recheck as your vet suggested.

I would not worry about the lowish neutrophils at this time. It’s not always that accurate and I would not think much about it unless it’s a continuing or worsening trend.

I think that’s everything.

I might try the WX in her mix of foods. She sounds like she is a food Diva and keeps you on your toes. Amdrew’s list is so helpful and hopefully you can get some different food options that are low phosphorus that Cleo will eat. You are smart, Angela, and I feel very good about Cleo getting the best care from you.
