Hello my name is Rachel and we have been treating my cat Jasper for diabetes for about 10 months now. I am feeling like I could use some guidance on dosing as it seems to me that his bg is a bit too high still and I really would like to get this more under control for him. Attached is his current spreadsheet. He eats fancy feast naturals pate 1 can at shot time in the am and pm and one can at +6. He also gets a topper of reveal at break and dinner because it helps to ensure he eats. I have been giving him 1.75 units of Glargine for the last few weeks( you can see his spreadsheet for more details) and today i reduced the dose to 1.5 units. I'm honestly not sure that was right but it seemed his numbers were just a bit lower on that dose a few weeks ago. He gets shots at 8:30 am and 8:30 pm. I test his bg before each shot as well as at +6 during the day and +2 at night. I would love some guidance because I just feel like we are having a hard time getting his numbers down.

Im having trouble with uploading a spreadsheet here. Can someone tell me what I am doing wrong?
 
Someone helped me to set up a spreadsheet on the Facebook group Feline Diabetes Support Group. I am not very familiar with using spreadsheets but once it was set up I have kept it updated daily. I am having trouble sharing it here though. It says I do not have permission.
 
Ah, from what I understand the FB group uses a different spreadsheet than this forum. Any chance you could create one here and populate maybe the last few weeks of data and share a link in a post to the FB group one?

One of two things is happening. We don’t know how low this dose is taking him at night nor do we know if he hits nadir earlier than +6 during the day. Cats often drop lower at night. Would you vary your second test during the day and get. +4 one day and +5 another? SLGS directions also say to do a 12 or 18 hour curve every 7 days or so. Can that be done over the weekend?

If not going any lower she is not getting enough insulin and has likely developed insulin resistance from staying on the same dose too long.

I can vary the testing, and i can do a curve for him early next week.

The FDMB Facebook group does, in fact, use the same spreadsheet as here. There are several other FD groups -- some have poached our spreadsheet and made alterations that often cause them to crash. You're good!!

A couple of observations. Lantus/glargine dosing is based on the lowest number in the cycle. Thus, it's important to get at least one test in addition to your pre-shot tests. In addition, many cats are nocturnal -- they can be more active at night. Even if they sleep through the night, some cats experience lower blood glucose (BG) numbers during the PM cycle. It's important to get at least a "before bed" test every night so you can go to sleep reassured that Jasper's numbers aren't crashing or that you're not missing a dose reduction. The +2 tests you're getting are helpful. If you are awake past the +2, like Elise (tiffmaxie) suggested, try to get another test.

With Lantus, we follow either the Tight Regulation Protocol (TR) or Start Low Go Slow (SLGS) for dosing. This is a link to the sticky note on dosing methods. One of the things you'll notice from the sticky is that the longest a dose is held is a week. Holding a dose that's not getting a kitty into better numbers means that their body gets accustomed to the higher numbers. When this happens, it makes it harder to get the numbers to get into a better range. (It's called glucose toxicity.) Given that you've had Jasper at the 1.75u dose for over a week, you may want to consider increasing the dose to 2.0u.

It’s really important you only post in one forum and I’d suggest you post in the Lantus, Levemir, Biosimilars forum as that is where the expertise is with glargine.

I believe there is also some issue with the SS. The link in your signature is the one I did but looks like @Virginie also did one for you? If you are entering data on that one, I’d move it to the one I did for you and delete all other SSs.

Hey Elise! She’s wanting to do TR. She has a condo on LBL from Friday. I am looking for an updated SS because I suggested she raise the dose. I don’t believe he’s dropping at night and it’s possible he needs another increase after just four cycles, depending on the data, because he is pretty high and flat and had DKA last Feb. We need to get him into some better BGs.

Can I ask if you see the updated numbers on this spreadsheet from this weekend? I am now confused because I didn't realize there were two made and I am not sure if I linked the correct one in my signature. Sorry I feel like I made this harder when two people made separate spreadsheets I just didn't realize that was happening until it was already done.

I don’t see them on the SS linked in your signature which is the one I did for you :)
 
@Virginie -- too many spreadsheets is a problem. In the future, could you reach out to one of the people who have taken on the task of helping out with the spreadsheet we use here? (i.e., @Bandit's Mom, @Marje and Gracie, or @Chris & China (GA) although Chris has her hands full with the FB group). We've had far too many problems with member's spreadsheets crashing when our template is edited. All of the people who have been helping with the spreadsheet have been doing this for years.
