Dose? Switching from vetsulin

Hello! I’m hoping to get some guidance on what dose I should start with when switching from vetsulin (1.5u) to Semglee.
We’re still pretty new to all of this so I’m not sure what info is needed other than his spreadsheet! He’s definitely not doing well on the vetsulin.

Thanks :)

Hi Kristy
First can you please add your signature so members don't have to ask you the sane questions over and over again tap on the blue link below help us to help you

I see on your SS you will be starting lantus tonight, I don't think members will see this in the remarks section, can you make a blank line above today's date and put started Lantus. And on your SS up top where you have vetsulin put 3-17 started lantus

You can put this in your signature also
I will tag a few members for you about the dose

Here is a link helping us to help you link. If you noticed, our members have some basic information about their cat's in their signature. This helps us to not pester you by asking the same questions (your cat's name, insulin type, date of diagnosis, etc.) repeatedly. We also have a link to our spreadsheet in our signature. We are very numbers driven. The spreadsheet is a record of your cat's progress. By linking it in your signature, we can follow along and provide feedback should you need the help.

Wondering if you need to start at 2U of Semglee since 1.5U of Vetsulin isn't dropping him below 300. I will tag the others again for their advice.

Are you feeding low carb wet/raw food? I thought I saw a mention of Pro Plan DM in your previous post.

Thank you!
How frequent should I test after dosing tonight?

He only gets fed LC wet with some raw here and there but he thinks he’s a Labrador and will eat literally anything from fruit to houseplants lol. I try to keep his diet as clean as possible but with three young kids around it gets tricky. Definitely no DM or kibble though

I guess you've seen this sticky on our dosing protocols:
Dosing Methods: Start Low, Go Slow (SLGS) & Tight Regulation (TR)

If you can get at least 4 tests a day (2 preshots and 2 mid-cycle), you could follow TR. It provides for dose changes every 3-5 days and can get Zissou to a good dose faster.

Even though TR only requires 4 tests per day, it doesn't mean you should only get 4....if it's possible, it's better to get tests at different points in the cycles so you can start to get a good idea of when Zissou's Onset, Nadir and Duration usually fall along the cycle. The idea is to understand his behaviour on insulin.

I went ahead and dosed at 1.5 for now just to be safe.

I can test as much as needed, he’s really good about it and my husband and I both work from home thankfully.
I’d love to try TR but wasn’t sure if it would be right for him. I’ll read over it more closely tomorrow.. Thanks!

I would start at 2.0 or at least 1.75. You need to get that bg down. Are you testing for ketones?

I did test the other day and it was negative. I’ll test again tomorrow with that

I’ll go ahead and do another .5u as well since everyone has recommended more. I definitely want his numbers down as quickly and safely as possible!

Sorry just seeing this. I would go with 2 units as well.
I would also test daily for ketones with high BGs like Zissou is having.
I would go with TR as you are at home and can test as often as needed. And you are feeding a low carb wet diet.
As we don’t know how he will react with the Semglee, I would test every few hours for the first couple of cycles.
Good luck!

Can you put a line (and colour it) between the Vetsulin and Lantus in the SS please so we can see at a glance when the swap over was? Thanks
