I didn't post yesterday as we had a boring day with pretty steady numbers and less testing since I was in the office all day.

I'm home most of the day today so will get a few more tests in today. Today is day 5 of insulin for us and I'm wondering about dosing, I know SLGS says hold the dose for 7days but his numbers are pretty steady so do I hold the dose or increase?

We leave March 17 for ten days and will be out of the country, our cat sitter is fine with giving insulin, not sure she's going to be able to test. With that in mind I'm leaning towards keeping him at 1u until I get back knowing his number are steady but also don't want to do something risky for him. No ketone in his urine and I have a blood ketone meter so can test. Any advice is appreciated.

I would suggest you follow TR and get him to a good dose sooner. After that you can always switch to SLGS if you think you cannot test as mucg as TR requires. Though we do have people who work and follow TR.
IS TIGHT REGULATION POSSIBLE WITH A FULL TIME JOB? YES!

It takes a week for the Lantus depot to build but it looks like he needs more insulin.

Tagging @tiffmaxee for confirmation on whether to increase him to 1.25U

On March 14th or so, we could look at whether you need a lower vacation dose for the time that you are traveling.

Okay good so bump him up to 1.25 tonight and then see what happens with his numbers and maybe go back to 1 for vacation dose?
The vet started him at 2u I went down to 1u after reading the forums but now am feeling a little guilty since his numbers are still steadily high.
I think TR is best for us long term just not sure to handle things with the timing of our trip, and trying to make a plan that means I can enjoy myself while I travel and not stress the entire time I'm gone. Thanks!

Yes, bump him up to 1.25U and re-evaluate. At his current sugar levels, you don't need a vacation dose. He is well above normal levels (50-100 mg/dl on a human meter).

We usually recommend starting at 1U, but he is big cat, so based on his weight his starting dose would have been 0.25U X 9.3, if following TR. For SLGS, a starting dose of 1U was okay. With TR, you can revaluate the dose every 3 days, so you aren't locked in to a dose for 7 days like in SLGS.

I just got off the phone with the vet and she strongly encouraged me to go up to 2u and monitor him closely over the next couple of days. Since 2u was her recommended starting dose and I got scared and went down to 1u, what do you guys thinks?

Sorry no one responded before. I see you shot 2U at PMPS.
See how he does on 2U. It's quite a jump from 1U to 2U. Close monitoring would be needed.

Thanks for checking in on us. I felt so spilt between my conversation with the vet and what I’m learning here. Ultimately I decided if 2u is a safe starting dose, I have a kit for lows, and can reliably test him I’m in a much safer place to do this than I was on Friday when I started. I’m also used to working nights so if I have to stay up feeding/testing him we’ll be okay… I hope

It makes sense to get him to a good dose before you leave on vacation. If he stays in higher numbers from March 17th, there is a chance of glucose toxicity setting in.

Glucose Toxicity sounds a lot worse than it is. When a cat stays in high numbers for too long, it's body gets used to higher numbers and treats them as normal. It is a kind of temporary insulin resistance and it then requires much more insulin to break through to lower numbers.

Thanks for helping me feel better about all of this. I’m going to take your advice on testing at 2hr and see which way we’re heading for the night (I saved that comment, it’s very helpful to have that insite). He’s had a normal dinner and seems happily himself right now.
