Benny AMPS 103 No shot .

Benny is showing some lovely numbers in blue and green now. However, as his curve is flattening out it meant he has a nadir close to shot time today. I delayed and fed him until +15 when he hit a yellow number so he has somewhere to go. That seems to have worked out as he is now 152 @ +3 after I shot.

It looks like the change to a lower carb food so that he is eating the same as Billy that you mentioned to me in a pm has lowered his BGs and he not needing the higher dose of insulin.
Areyou testing everyday still for ketones? I think this is important especially as you have reduced the dose of insulin.

How is he generally?

He is completely back to normal! He is active, eating well, playful. I am so thankful to you all here. I am testing for ketones but not everyday. He has started using the same tray as Billy so I need to be "present and armed with test strip" I will try harder! I adjusted the SS to add some numbers in at +12-+14 because I thought that information was important. I am wondering if he might be a once per day dose rather than twice? It is starting to have a long effect on him, or maybe the changes in dose times has interfered with the overlap?

I would still test daily if you can please, as this test is so important in telling us if we have everything right (food, insulin, fluids) especially while he is changing /delaying doses at the moment.

Lantus is not a once a day insulin for cats as they have a fast metabolism. We just may need to adjust the dose so you can shoot both am and pm doses. This is a common issue as cats start to need less insulin. I’m talking to someone about it and will get back to you.
 
Hi Fiona, I have spoken to some of the other experienced people to see how we can help you get Benny into better numbers following the DKA/ketones and to also make it a bit easier for you while you are having some issues at home.
These are the suggestions
  • Go back to doing TR but use 70 as a reduction point.
  • Post daily for suggestions and to keep us in the loop and keep the SS updated please. Putting information into the remarks column is very helpful.
  • We need to find a dose that you can shot twice a day that doesn’t put him into the 50s. So stay with the 1.75 unit dose and see how that works out.
  • You need to make sure you are catching the drops early so that he is not skyrocketing afterwards and then dropping low again. To do this, if you get any number above 200 at PS that looks like a bounce is going to clear, ie the BG is not on the way up and he has been in higher numbers, you need to make sure you get a +1 to see if he is already dropping and if he is you need to start feeding the curve like we have discussed before. Test again at +2 to see where he’s at. You want to stop giving him a reason to bounce and those reasons are dropping too fast, dropping too low or dropping lower than he thinks is normal for him.
  • If you are unsure about anything, post and ask for help until you have learnt how to manage. Don’t just go ahead and guess.
  • I know you have recently started giving him lower carb food so he’s eating the same as Billy, but there may be times when you will need to feed him the higher low carb to keep him from dropping low or to keep him in low to mid blues
  • Shoot slightly reduced doses when Billy’s PS is lower and you are tired….as long as there are no ketones.
  • Test daily for ketones and you need to be aware of any indication of him developing ketones or DKA such as not wanting to eat or acting lethargic.
You will need to fix up the SS for 1/8 where you gave a late dose. If you can’t fix it yourself I will ask @Bandit's Mom to have a look at it.
How does this all sound?
you will need to change the signature to say ‘TR custom dosing. Reductions at 70. Do not follow this individual dosing’

Sounds like a fantastic plan! I have copied this onto phone for easy reference. I am not sure how to fix the SS. I wanted to have the numbers +13 to +16 for reference and to see how long the shot was active for, but wasn't sure how to do that.

@Bandit's Mom is a whizz at fixing SS. I’m sure she will ne along sometime soon.she may need to send you a pm so keep an eye out for it.

That is quite a big drop from +2 to +3 tonight. Did you give him some food at +3 to slow the drop?
I think you will need to watch the +1, +2 and +3 for each cycle at the moment and feed him at the first sign of him dropping more than 50 or 60 points (2.8 or 3.3) in each hour.
He is starting to be in much better numbers but still likes to drop and bounce. We need to take the reasons he does this away from him. I did that with Sheba.
As you are using a freestyle libre, it should be fairly easy to get the tests. Is that doable for you?
