Urgent Benny 232 AMPS should I shoot?

Benny is a DKA kitty of 4.5 weeks. He has had a couple of hypo events. He has always been high preshot and these occurred when he was in the pink pre shot. Today he is in the yellow! Dose currently is 2.2.

Hi Fiona, yes I would give the full dose. You have shot a 257 before. Which is not much higher. When the Preshot is lower the BG does not drop the big drops that they can when the Preshot is much higher.
Just monitor the cycle and get a +1 and +2 to start with to see what he’s doing…. and give him a snack at +1 and +2 then feed as usual.

Look at this as progress. I know it’s scary when you start shooting lower numbers but you will eventually be shooting green Preshots. Lantus gives lovely flat curves once the Preshots come down.

I will have to get Valium! Their regular Vet said yesterday that it can take months to get stable curve sometimes. I nearly burst into tears! She is lovely and has known them since kittens. She cuddled Benny for ages!
 
It can take a while to get regulated but as long as he’s well, that’s the main thing. Some cats just suddenly fall into line and read the rule book. When I get a chance, I’ll link a few SS to show you. Don’t stress about it though. Benny is so much better than he was a month ago and that’s what’s important.

No, I think I will stick with Libre sensor for a while. Benny is not fussed about having it. I might get Vet to show me best way to use the blood monitor next time though. I have watched videos but when I did try it, it was awful for Benny.

You can prepare him by picking a spot to always test him, take him there a few times a day, rub his ears, and click the lancet device to get him used to it, then give him a small treat every time you do it. He will soon associate it with a treat. Sheba used to come running from wherever she was in the house when she heard me at time testing equipment as she knew she would get a treat.
I see he had a little food bump at +1. Looking good..

How are you liking the Freestyle Libre? We are getting one tomorrow.

I think they are brilliant, this is his third one. Although the glue does give the equivalent of a good waxing. Benny had a hypo in hospital and so they were quite keen for the sensor for a few weeks. This one the Vet said it was up to me. Benny is not bothered at all.

Will you keep using it or is this just to get him to the proper dose? Does it stay put for the whole 2 weeks?

I think I will use until he is stable. It stays put the whole 2 weeks and is super easy to use. I use my phone app to scan it. It means at night I can set the alarm, quick scan and go back to sleep. I also have a trip planned next year and I think I would have my husband use it. The "needle" that goes into skin is more like a fake Christmas tree needle , like plastic.

He is nearly there! 11.6 @ +4. I have been looking at other peoples spreadsheets and although I am terrified I think with your help I would like TR. I do not think that he had been diabetic that long before the DKA. It was most definitely the switch to a dry kidney diet (along with Temptations creeping back into use just before the DKA)
 
You have made a good choice. We have basically been doing TR up until now because of the DKA. You will manage it well. I would print off the TR method as you will likely refer to it a lot. This will give him the best chance of remission. No guarantees but the best chance.
 
@Bron and Sheba (GA) Benny has dropped to 6.7 at 2.5 hours. (from 22.9 amps) how closely do you think I should watch? That is quite a fast drop!
Did you give him a snack at +1 and +2? If you have not already done so give him a good snack of low carb.
I would test again at +3 and see what he’s up to.

Yes offered a snack but not really interested. Ate well during early morning and night (food left out gone) Given him a few temptations which he has eaten. Will test at +3 again. Thank you @Bron and Sheba (GA) for the speedy reply!
Try and get him to eat something. We want to slow down that drop. He obviously chose the fast slip slide this morning……Benny, please take the sedate slide.!:cat:
 
His appetite is pretty good tbh. I think my problem lies in feeding ad lib and tight regulation. I will try and sort that out but it is difficult having Billy (who muscled right in on the gravy lover btw!) who has his needs too. Will have to figure out the best schedule. We have done a lot of changes in a short period. Perhaps the switch to wet food and increase was a big factor?

I would give him a bit whenever you feed Benny. It doesn’t have to be much. Just so he knows he’s not missing out. If I feed one of my cats and not the other, they look at me as if I have two heads.

The change over to the low carb diet has made a bit difference ( you can see what higher carb food does to the BG when we give it to raise the BG. Imagine feeding it all the time……a lot of people do, and most likely because they don’t know about high carb food and what it does to cats BG).
Also you have found a good dose of insulin at the moment. We just need to convince Benny not to bounce….:rolleyes:

I would continue to do the +1 and +2 snacks to try and slow down those big drops. It’s hard to know which cycle he’s going to drop so I would just do it routinely at the moment. If we can get him down into better numbers in a more sedate way, he is less likely to bounce. And checking on the +1 and +2 will give you a heads up what’s likely to happen and you can intervene if needed.
He’s starting to get more blue and green numbers so his body is getting more used to them again which is good. It’s the big drops like today that will most likely cause the bounces. Once the big drop has happened, it’s too late and all you can do is stop it dropping further. The bounce will happen.

Does he mess with the sensor much? Wesson is grooming around it and licking it. Do you use a shirt or covering for the sensor?

No he has totally ignored them. He lies on them to sleep. They don't seem to bother him at all.
 
@Bron and Sheba (GA) would you have a look at Beeny's SS for me please? My husband and I looked at the syringe with magnifier on phone and saw that I had bubbles in the dose. We then thought that perhaps that is why he had been bouncing around, bot getting regular stable dose. So we gave a 1.5 and then 1.8 , then last night I did a total furshot, I think that when I pulled back to check vein it must have come out. So, he has been high all night. I then brought his AM shot forward by 2 hours and gave 2.0 but smelled insulin again ( could this be from last night?) he is still High at +4 !!! Negative ketones but very thirsty overnight. Eating well. DO you think I should call the vet and ask about getting him some short acting insulin?
 
He’s high because he’s not getting enough insulin. Go back to 2.25 units next dose. Not sure why you reduced to 2 units on pm dose 18/12.
keep testing for ketones.

Hes drinking more because he needs that 2.25 units
Don’t worry about some tiny air bubbles. No I don’t think he needs a short action insulin. don’t try and draw back before you shoot.

When you draw up the dose flick the syringe with your finger to get the bubbles to the top then get rid of them. Then correct the dose if needed Tiny ones are often left behind. Don’t worry about them. You don’t push air into the cartridge when you draw up do you?
the depot needs to fill again now because of the reduced doses and FS.

BTW On LLB you start a new thread each day and put date, name of cat, AMPS and then question if you want eg 12/21 Benny AMPS HI..

I will start a new thread but just wanted to ask you about injecting air into the vial. Lantus instructions say to do this an I notice all my syringes come with 1-2 units of air in them. Are you saying I should not inject air into the vial? Benny lost his sensor today and had been so High the night before, he was 12.6 PMPS so I am watching very carefully. Sorry to ask you a question on Christmas Eve. Merry Christmas!

No don’t put air into a cartridge. Just expel the air then draw up the insulin.

I noticed you said you gave the insulin 2 hours early this morning. If you do that you are effectively giving an increase in dose. The most you should move the time of the dose is 15/20 minutes each 12 hours unless you have skipped a dose in which case you can move the time forward without a problem.
Did you get the libre replaced?.

Thanks. I realized it was effectively increasing the dose as the Vet said to try 3.0 which is too high. I wanted to move his shot to a time which suited his eating habits and as he had been High for 2 cycles I thought it would be ok. I thought he might go lower today as result and as he lost his libre sensor he was going to vet. I got them to do his blood ketones as well. I thought his breath was a bit fruity ad well as wet food pong!
 
Don’t put the dose up to 3 units. The dose is ok at 2.25 unit. He’s just bouncing from the lower numbers still and the days you dropped the dose also means the depot drained and the BGs went up. It needs to fill again and stabilise.
If he doesn’t drop to green soon we may need to increase to 2.5U. I don’t like all those black BGs.
I look at the SS every day. Can you keep it updated please?
It would be good if you could post daily if you can manage it as then we can commentary .
