I thought SLGS would be a good fit in the beginning, but it became apparent I need to start shooting lower numbers or we weren’t going to get anywhere. So the past couple days I’ve been trying to collect data giving him the smallest dose I can just to see how it affects him… seems there is still room for him to go lower but I’m looking for dosing advice as well on guidance as far as switching methods to the modified prozinc method…

willing to do what it takes to try for remission.

Hi Jackie, I am not a Prozinc user so I will tag @JanetNJ
One thing I would do is always get at least one test in during the pm cycle as cats often drop lower at night and the pm cycle is just as important as the am cycle.
Are you feeding Mickey snacks during both cycles as well as the main meals?
He is certainly looking great and I think you have a great chance of getting him into remission. But you want to make sure it is a strong remission and to do that you keep the insulin going for as long as possible to support the pancreas.

I think MPM is a great choice, good chance of remission in my opinion. I would stick with 0.1U for awhile to see how he does. He doesn't appear to go too low on it, I believe you can safely shoot anything over 100 at 0.1U as long as you are around to monitor ( we may gradually lower that number as you get more data). Just to double check - he fasts 2 hrs before preshot correct?

You would reduce the dose any time he goes below 50 since you have a human meter. From 0.1U you'd go to a drop dose - push plunger all the way down, insert in vial, release to get one drop in the needle, not visible. When administering, you hold the plunger down for 10 seconds if the cat will let you.

If you get a preshot from 90-99, you can also try a drop dose if you can monitor. I think giving him insulin consistently will help him level out nicely into greens, it's the skips that are sending him a little higher and inconsistent pattern

I think you're doing a great job. I like the 0.1-0.25 doses you've been doing. I'd aim for a low of around 70 - 85

As far as snacks/meals go… he likes to munch on small amounts throughout the day so the only time I pick his bowls up is 2 hours before his pre-shot test.

I skipped tonight after getting 104, stalling 30 minutes and then a 96. A drop dose probably would have been better

so I skipped the pm shot tonight after getting a 104, stalling 30 minutes, then a 96. So maybe a drop dose would have been better. I remember being told that stalling is to see which direction he’s heading, so I’m wondering if he is going down then should I still shoot or only if he’s going up???
And yes I take away his food bowl 2 hours before shot time. Other than that, he snacks all day and never really eats a large amount at once.

So the 96 and 104 are within meter variance, I'd have considered them the same number and gone ahead with the drop dose (basing dose on lower number just to be safe).

I would however reduce your standard dose to 0.1U, I think that gives you a better chance at shooting a consistent dose both AM and PM (I know others said 0.25U looked good, I just prefer to start lower and see if that gets consistent shots, then take him up if needed)

Next time you get a preshot that's below what you're comfortable shooting, consider stalling without feeding for up to an hour and see if the number is going up.
