Good luck with the increase :). The fat/skinny doses really work best when you are getting close to a good dose, and seeing good regulation, but more often than not 0.25u increases are the way to go. You might want to post your other question on the health forum as well, in case there are some folks there with some experience.
 
My cat had clostridium perfringens alpha toxin.

Did they have to do a biopsy to find that? After taking the Tylosin did he get better? I've read that SIBO is very hard to diagnose and just as hard to treat. Sounds like many kitties end up on antibiotics for the rest of their lives which as you know isn't good either. :(

No biopsy. They just sent the stool out for some expensive testing to find out the pathogen. He took the Tylosin for a few months and I did give probiotics. He had the worst unresponsive diarrhea and it smelled very weird, almost fruity or sweet (in a disgusting kind of way.). What are Koda’s symptoms like? Clearly something led you and your vet to do the GI panel.

That's what's weird. Koda rarely ever has diarrhea. All my kitties tend to be on the constipated side if anything, which is strange because they eat predominantly wet food. Sometimes his poo is REALLY pale and super stinky. Sometimes it's unusually large (diameter). The biggest thing was that he was losing weight pretty quickly. Sometimes his appetite is very little and other times he wants to eat every hour. He goes in spells of vomiting which could be the pancreatitis. I can't believe I still haven't gotten the TLI results back. It will be a week on Monday. Based on the fact that his B12 & folate were elevated makes me want to go ahead and start him on the Metronidazole I have had in the cupboard since July from a different vet. I thought it was just to help stop diarrhea which he's never had so I didn't give it to him. I was afraid of it making him more constipated. Now I'm thinking that was dumb and maybe he wouldn't have lost this weight if I would have just done what that vet wanted. :(

Yes. I think you might want to try the metronidazole! The pale stools point to a pancreatic problem. But you already know that.

I started Koda on proviable-dc on Friday. Last night I went ahead and gave him one of the 50mg metronidazole's. This morning he doesn't seem to feel well. I had to follow him all over this morning to eat half of his food and he has been drinking a LOT of water. I read that metro can cause nausea and inappetence. I can't really afford this right now as he's already lost weight too quickly. I did not give him any this morning and am just going to hold off until the TLI result comes back. I think an ultrsound might be in the near future. :(

Poor boy. I agree. If I had a kitty losing a lot of weight like that, I would do an abdominal ultrasound. My kitty that had the clostridium perfringens alpha toxin ended up (later on) being diagnosed with small cell lymphoma in his GI tract. We first did Prednisolone a d later had to add in chlorambucil.

That is my fear, lymphoma. How long was if after his diagnosis? Did he pass on his own or did you have to make the call? How old was he? :(

It was probably a year later that we discovered it He did pass away on his own at home. He was 16. He had a great three years or so on Prednisolone and Leukeran (chlorambucil- the chemo drug). He was like a new cat and gained so much weight back (he was a large cat). He ate well and was happy and normal again.
