I'm going to use Max2 in my title posts to help alleviate any confusion with the other Max here.

Today was my Max's first test and first insulin. It went pretty well, although I've actually been stuck more times than Max so far. My eyesight has really gone down since I last had a diabetic kitty. I guess I'm going to have to add a magnifying glass with a light to my testing area.
 
Hi. Is this your first post about Max? If so you need to post an intro in main/health I think. Since you were here before with another cat maybe not. In any case welcome. I love your cat’s name! That is a large starting dose for his size. Do you think his diabetes is steroid induced? Since you are following SLGS and feeding lc wet the starting dose would be .50.

Oh, I knew I was missing something. I'll pop over and do an introduction after this post.

It could be steroid induced. He's been on budesonide long term for his IBD and the last 3-4 months he's needed it more often.

Regarding his starting dosage....my vet had wanted me to start a 2 units and it wasn't until I did his amps that I thought maybe I should start at one. I decided to give the 2 units three days and then will re-evaluate. I think ultimately I will be doing TR protocol. But I agree, it seemed a little high to me as well, but knew I'd be doing lots of testing just in case.

I did look at the SLGS protocol to refresh my memory and thought it didn't really address Max's situation, but I would love some input. It says:
  • 1u BID if kitty is not on a wet/canned low carb diet - He is.
  • 0.5u BID if kitty has been switched to a wet/canned low carb diet - He did not switch. He's been on the same diet for years.
So it doesn't have a recommendation for a starting dosage for cat who is on low carb wet food, but it wasn't switched. Hmmm...confusing.
 
Think the part about being switched is because most people that follow SLGS feed dry food or recently switched. Since he’s always been on lc and given his weight if he were my cat I would have started with 1.0. With TR. it would be .85 so either .75 or 1.0.

Welcome back (?) Stephanie and welcome Max. I definitely needed magnification so I got the glasses with a light so it’s hands free. Purchased on Amazon. If you want the name etc, let me know, they have lots of different styles. Wishing you both all the best as you begin your diabetic journey with handsome Max. :):cat:

I'm having a big debate with myself. I think I'll .....I've changed this sentence several times. Argh.

If you're thinking about switching to TR, the initial dose is based on weight. The formula is: starting dose = ideal weight in kilograms (or actual weight is under/overweight) x 0.25. In Max's case, the dose would be between 0.75 and 1.0u. As Elise noted, 2.0u is a large starting dose. Since he's already on a canned food diet, with SLGS you would start at 0.5u.

Also, remember that it will take 5 - 7 days for the insulin depot to form and stabilize. You may not see much movement in Max's numbers until the depot is in place.
