Hello! I am a long-time lurker but first time poster

Millie, our 16 year old Burmese, was diagnosed on 13/05/2021 as we were concerned about her loss of weight, lethargy and bad breath.

After the initial shock following the diagnosis had calmed down enough for my sensible head to kick in, I spent 3 days solid reading about the subject, trying to get my head around what we should be doing to best support Millie. I immediately changed her diet to low calorie wet food, found some suitable low carb treats, found a better litter system to help with the excess peeing, researched the need for home testing and bought the machine.

We were initially prescribed caninsulin, but we spent more time not shooting her due to low AMPS than we did giving her insulin. We changed over to Prozinc which is better, but is still not working well for her.

She was diagnosed with DKA in July, following a UTI. She was in hospital for 5 days. It was the worst 5 days of our lives! Although she has made a remarkable recovery (she is putting on weight, eating well, peeing less, still drinking but significantly less and her coat is super soft again) it has left me ultra-vigilant with constant anxiety about her well-being and her BG readings.

I have had long discussions with our vets but she feels this is the best we can do for her, despite the fact her numbers are, as she described, “all over the place”. I have been pushing to be moved to Lantus for a few weeks now, but the vet has no experience of this insulin so wanted to review the empirical evidence on Glargine. I was very impressed that she liaised directly with a FD specialist who reviewed Millie’s spreadsheet last week. He agreed with me that Millie burns through insulin too fast and Lantus would provide better glycemic control without the dramatic drops/rises that Millie has on Prozinc. So that is where we are at currently. My vet is on holiday, due back in a week, when we will have a discussion about moving her to Lantus in the next few weeks.

Welcome to the forum Karen, or do you prefer Miggins? Welcome pretty Millie :cat:. You’ve certainly done a lot of homework :bookworm:. Great job getting Millie on a low carb diet. Congratulations on finding a vet you are happy with and trust. Good luck with the change to Lantus in the next few weeks. You best bet would be to buy the 5 pens or cartridges, they will last you most likely for a year
You would use the pens like a vial, just insert the syringes into the rubber stopper and draw your insulin. The syringes will have half unit markings so you can increase or decrease by 0.25 units, if you were to use the pens you can only increase or decrease by whole units

Welcome to FDMB! Some cats do better on Lantus than on Prozinc. We have a few here who have made that transition.
Is there a reason your vet wants to wait a few weeks to make that transition? With a history of DKA, you want to get Millie regulated sooner rather than later.

Your spreadsheet is in the format used by another FD group and it crashes on some phones/tablets. I can help you with moving your data to the format we use. It has both mg/dl and mmol tabs. Most of our members are from the US and it is easier for them to be able to see BG data in mg/dl rather than mmol!

Is there a reason your vet wants to wait a few weeks to make that transition? With a history of DKA, you want to get Millie regulated sooner rather than later.

Your spreadsheet is in the format used by another FD group and it crashes on some phones/tablets. I can help you with moving your data to the format we use. It has both mg/dl and mmol tabs. Most of our members are from the US and it is easier for them to be able to see BG data in mg/dl rather than mmol!

thank you for sharing this information!! I am also on the UK feline diabetes support group on Facebook. I believe some of these members are on here too!! They have been helpful with sharing information too! I purchase my U40 syringes from vetuk already, so I know they are a good company!

I am going to speak with our local pharmacist this week to see what info they need on the prescription so when my vet is back we can sort this out ASAP.

Thank you! I am really hoping Lantus is better for her. My vet is on holiday this week, back middle of next week so that is the earliest I can get a prescription sorted out for her. She wasn’t too keen to prescribe a human insulin, even though I could show that Prozinc wasn’t working for her. It has taken me a number of weeks to get this far. I also need to speak with our local pharmacist to check what info they need on the prescription and order the right needles.

Re the spreadsheet, I am sorry for the confusion. My husband had to set the last one up for me, so if you could help I would be grateful.

With a history of DKA it is important that you are testing frequently for ketones. If you could put the results of any tests into the spreadsheet remarks column, that would be great thanks.
Also make sure Millie eats plenty of food during the day and evening and don’t skip any insulin doses. Do you have an automatic timed feeder for food overnight or when you are away from the house?
I agree that Lantus may be a better insulin for Millie. We need to get rid of those high BGs..
Please try and get some tests in during the pm cycle as cats often drop lower at night. Getting a before bed test in is a good habit to get into. If the before bed test is lower than the preshot BG, I would suggest setting the alarm and testing again later to see how low the BG has gone.

Hi! Thank you for the welcome message! We do test for ketone using the On Call GK dual meter (the same one our vets use). I bought it as soon as she was diagnosed with DKA. The readings are noted in the remarks column already.

Luckily I am at home all of the time so fed her on demand. She is a very good eater…not fussy like my other two! On the rare occasion that we go out, I have two automatic feeders but despite showing them where they are and what is in them, none of them eat out of them whilst we are out!! That’s Burmese intelligence for you!!

I am hoping Lantus will help flatten the highs and low that she has experienced with the other two insulins. I feel her body is constantly fighting against the insulin all the time. We can but try!!

There is nothing i can add as to where to get your needles, insulin etc as we get all ours direct from the vet and the insurance sorts it out.
I do hope that the vet gets Millie onto Lantus asap. There are a lot of experienced members on here that know a lot about Lantus and will guide you through everything.

Thank you for taking the time to answer! Unfortunately we have never had insurance on Millie. However up until now she has been a very healthy and well cat. The DKA was a very expensive (and scary) experience…one hopefully we never ever repeat! She is doing much better now. Just need her on an insulin that stays in her system longer than 8 hours and will managed her numbers better….no pressure Lantus :D

I do get my syringes from all day pharmacy, the linka that Bhooma gave you. I get lantus from the local pharmacy. Is a bit funny but completely straightforward.

I see you use gluci navii, and I do too. Have found multi buying on ebay the best deal, and for lancets I just get the cheapest on Amazon ( we also don't have insurance...)

Hi! Thank you for your lovely message. It’s very kind of you to take the time, especially as you are so busy!

I am pretty much all set up ready to go, bar the actual insulin itself!! My vet has just got back from a 2 week holiday. I am going to chase again today. She has been really good about speaking to a couple of FD specialist about moving Millie to Lantus but she would prefer me to stay with Prozinc despite seeing the high numbers and the fact it is out of her system within 8 hours. We really do need to try her on it.

I bulk buy GlucoNavvi test strips from here. https://homehealth-uk.com/all-products/glucose-navii-blood-glucose-test-strips-50-strip-pack/. There is a good deal on 10 boxes.

I have phoned all the local pharmacies to find out the best price for the insulin - quite a big price difference!

I’ll join the Lantus forum once I have the prescription.
