Hola! Soy la mamá de Nicolás, la desesperación y la angustia me han traído hasta aquí, y espero poder ver un poquito de esperanza gracias a vosotros.
Nico debutó con diabetes el 10 de junio, desde entonces hemos pasado por Prozinc, Lantus y Toujeo, consultas en veterinarios, ingresos y especialistas. Pinchazos en las orejas y muchos sensores Freestyle
El caso es que no sé si nada de esto ha servido de algo hasta ahora...
Y ahora gracias a vosotros me planteo de nuevo un cambio a Lantus, dosificando más correctamente Cuando estuvimos con Lantus fue una locura, cambios de dosis continuos, Nadires en las tres primeras horas o ningún cambio tras ponerle la insulina.
Quiero agradecer la entrega y el apoyo recibidos durante estos días por parte de Bhooma y de Gill (lo siento, no sé etiquetaros) sin ellas ayer me hubiera hundido para siempre, gracias por no dejarme sola y por vuestro apoyo chicas

You are in excellent hands with Bhooma and Gill. Listen to what they say. Your kitty is in good hands. They have helped me with my cat, Phoenix.

Gracias Scedal! ¡Ojalá! La verdad es que me dan miedo los cambios, porque justo hoy he conseguido una curva aceptable con Toujeo, pero ayer se desplomó, no llegó a hipoglucemia grave, pero a Nicolás las bajadas bruscas le sientan fatal aunque no haya hipoglucemias. Espero que con la atención de Bhooma y Gill no las haya.

Después de hablar con varias compañeros sobre la situación de Nico, quizás sea mas prudente empezar con 1.5U de lantus mañana en vez de los 1.75U de los que hablamos. A veces cuando cambiamos de una insulina a otra pueden tener una reacción fuerte, en el primero ciclo. Para intentar evitarlo, si empezamos un poco por debajo no pasa nada y siempre podemos subir la dosis si vemos que los niveles no caen en la franja que es nuestro objetivo.
Así vamos con mas control, sin locuras:)
También estamos teniendo en cuenta que si le cambias el pienso a uno bajo en carbohidratos, eso es probable que reduzca la cantidad de insulina que necesita y si intentas poco a poco quitárselo del todo (seria lo ideal), eso se lo reduce aun más.

Dime cuales son las latas que come? Sabes que son los porcentages de carbohidratos que tiene?. Ya se que no le das nada con cereales, pero a veces le añaden azucares o verduras (como guisantes/batata/arroz) que influyen el porcentaje de carbohidratos. Con Diabetes estamos buscando tener menos de 10% carbohidratos.
Muchas de las comidas que son aptas para Gatos diabeticos estan en la tabla en este enlace

Si no sabes el porcentaje de carbohidratos podemos calcularlo.

Aquí está el enlace para la tabla de Nico, puedes ponerla en tu firma (signature) junto con otra información importante, como el tipo de insulina, problemas de salud, cuando se diagnostico con diabetes, para evitar tener que estar repitiéndote. :) Te voy a mandar un mensaje personal explicándote lo de la firma para que puedas hacerlo cuando tengas tiempo.

After talking to several members about Nico's situation, it might be wiser to start with 1.5U of lantus tomorrow instead of the 1.75U we discussed. Sometimes when we switch from one insulin to another they can have a strong reaction, in the first cycle. To try to avoid it, if we start a little lower, nothing happens and we can always increase the dose if we see that the levels do not fall in the range that is our objective.
So we go with more control, without crazy things :)
We are also taking into account that if you change the dry feed to a low carbohydrate one, that is likely to reduce the amount of insulin it needs and if you gradually try to remove it completely (it would be ideal), that reduces it even more.

Tell me which cans does Nico eat? Do you know what the percentages of carbohydrates are? I know you don't give him anything with cereals, but sometimes they add sugars or vegetables (like peas / sweet potato / rice) that influence the percentage of carbohydrates. With Diabetes we are looking to have less than 10% carbohydrates.

Esta mañana Nico cambio de Toujeo, a probar de nuevo con Lantus.

Dosis 1.5u.

Tuvo una mañana interesante, Nico decidió de tener un ciclo muy activo bajo de 379 a 65 en tan solo 3 horas, parece que se canso de botar por la noche y nos dio una sorpresa, usando un poco de pienso seco, bebida alta en Carb, y un poco de sirope de agave, conseguimos que se quedara en niveles seguros.

Por ahora Nico come algo de comida seca, y por tanto tenemos que tener en cuenta las normas de SLGS, (se reduce la dosis a niveles menos de 90) así que en principio, se indica una reducción de dosis a 1.25u. Mi duda es si es apropiado en este momento dado que sabemos que cuando cambiamos de insulina pueden tener una reacción fuerte que luego no se mantiene. Nico no ha tenido DKA, pero tiene rastros de ketones en la orina, y sufre de infección crónica del sistema urinario, el apetito bien hoy.

Que piensan? Teniendo todo en cuenta,
  • mantenemos vemos si sigue esta tendencia de bajar por debajo de 90, o si solo fue una reacción fuerte al cambio de insulina?
  • o directamente bajamos la dosis a 1.25?

Unfortunately, I have only seen a couple people switch from Toujeo to Lantus so don't know what to expect. Cats on Toujeo is very uncommon. It might be easier to drop to 1.0 unit.
Does Nico have a heart condition?
 
Nico has a hypertrophy of the left ventricle, diagnosed with echocardiography the same day that he debuted with diabetes. No treatment was prescribed.

Parece similar a la miocardiopatía hipertrófica que tenía mi Neko. No noté ningún problema con las caídas de azúcar en sangre.

Seems similar to hypertrophic cardiomyopathy that my Neko had. I didn't notice an issue with blood sugar drops.

Would I have to worry about ketones when lowering my insulin dose? or being a gradual descent and having Toujeo deposits still in your body, no problem? I read that when the insulin dose is lowered the ketones can go up, but I interpret that this is because of the probability of reaching high glucose values ... but I don't know ... it escapes me. Thanks for your advice Wendy.
