8/20 Ruby subQs lower BGs? AMPS 175/+4 59/PMPS 337/+3 256

I returned home this evening from a two day work trip. DBF and the sitter took care of the kitties while I was away and everyone was fine and hungry when I arrived. Ruby still has not pooped yet, and I asked the sitter/vet tech to give her fluids this morning during his +4 visit as I don't want her constipated again and it's been three days since the enema and no sign of poop despite a .5 ml twice daily dose of Lactulose. When he called me today with the 59 at +4, I told him to give her an ounce of HC because I was not going to be home until PMPS (possibly why her PMPS is so high, or it's a big bounce), and decided I did not want her to get fluids as I believe when Ruby dipped low after her oncology appointment the other day it was because she received subQ fluids in the office. I did not want to leave her home alone without being monitored after fluids. We will do the fluids tomorrow after her acupuncture, and only if her BGs are high enough. We will see if her BGs plummet again after the fluids. If they do, then I might ask him to give them to her every time he is here testing and feeding her and then lower her insulin accordingly.

I believe some CGs here have some experience with subQs lowering glucose levels. I only worry that it will happen when she can't be watched all day. Her current 2.5 unit dose leaves me feeling really nervous about her being home alone for 7 hours from the time the vet tech comes to the time I get home everyday. DBF is going to be away all of next week so I'm not sure how to handle this situation.
 
Fluids lowering BG is an ECID thing, I did notice it once with Neko when she got fluids early in the cycle before her Lev onset. And I had shot shoulders that AM. Typically I tried to shoot flank or away from the fluid site, and I also did it in the last half of her cycle when her numbers were going up anyway.

I too would be worried about this dose if you are away, since she did go under 50 on it. It is not what we’d suggest for a pet sitter dose.

I hope you see some litter box action soon. :bighug:

Thanks, Wendy! The sitter comes once a day only to test at +4. DBF and I are always giving her shots and pilling at AMPS and PMPS (we have coordinated all of our travels around Ruby). As you can see on her SS, he comes whenever DBF and I are working, and DBF's schedule is inconsistent because sometimes he's needed at the office and sometimes not. She seems to have really responded to 2.5 units, as she was in so many yellows on 2.25. This coming week, DBF is traveling and I will be gone from +1 until +11.

What would you suggest as her dose if we give her fluids this week?
 
And I meant to suggest you ask the acupuncture vet if there is some acupressure or other techniques you can do at home to help her motility. I had daily acupressure I did with Neko to supplement her acupuncture treatments.

When we start doing the fluids regularly I'll shoot flank rather than shoulders.

Ruby's no stranger to fluids. See 2/11 and 2/18/21 on her SS. The big difference is that she was tightly regulated then, and it also was not as hot as it is now. I have no doubt that she is dehydrated despite the water I put in her food, so she might be responding to the fluids now more dramatically. It's a bit hard for me to know. I guess we will see tomorrow.
 
The dose of lactulose seems quite low - I wish I could help you and Ruby! We started with 4ml twice daily, then tapered down for a few days. That was after her manual extraction. She was 12 lbs then. I know the situations are different.

Tina started subQ solution a couple months before going OTJ, and I never saw changes in BG patterns.

In increased Ruby's Lactulose to 1 ml this evening. I'm trying to avoid diarrhea, as that's what happened last time I started her on 1 ml of it. As her oncologist says, managing constipation is an art rather than a science, so I'm trying to methodically find the best dose for her by starting slowly. I'm glad to know that fluids did not affect Tina's BGs. I really don't want to have to dose Ruby's insulin around the prednisolone and the fluids. Just too many variables. It makes my head spin!

Ruby's primary DVM wanted to start her on 2 mls of Lactulose, but we were worried about diarrhea so split the difference and did 1 ml. She got diarrhea and the scoots after that so I stopped the Lactulose completely.

I'm going to get some chicken broth tomorrow and leave it out for the cats instead of their usual water that they ignore. Maybe they will be more inclined to drink chicken flavored water rather than the plain kind.
