? Is this what a reset looks like?

Hi

Previous post

Lily started bouncing and it didn't resolve. I realised when I started using calipers that her starting dose and all subsequent doses had been larger than I thought (probably not quite as large as I have now got on my spreadsheet but somewhere between 0.5 and 1iu larger). She had some low reads and I was dropping her dose, any bounces were resolving in well under 24 hours, but then the long bounce happened and I started worrying there was something underlying that had flared. I took advice and 'reset' her on the planned starting dose of 0.5iu. I'm on day 5 of that dose now and she's still very high and I'm feeling terribly guilty. I haven't been testing a lot during her cycle outside of the PS tests because I was waiting for her to settle but have started taking spot checks now. Hopefully there's enough information for someone to guess at. Instinct is of course to increase her dose because insulin will help but given her bounce I'm not sure. Shall I hold it for another few days?

Thanks

Liz

The "reset" has only left her in higher numbers. She needs more insulin and allowing her to remain in higher numbers could potentially lead to glucose toxicity.

Bouncing does not mean that Lily is getting more insulin that she needs. It just means that her body is unused to the lower but normal numbers that she was seeing. Bouncing usually resolves with time (although some cats are bouncier than others). When you increase the dose in 0.25U increments and test enough to see how low the dose is taking Lily, the chances of overdosing are nil.

I would suggest that you take her back to her last "good" dose which was 1U

Hi Elizabeth,
I’m not sure who told you to “reset” Lily but I am sure it would not have been anyone on this forum or our sister Facebook page.
All resetting will do is put Lily into higher numbers again and risk ketones developing.

Resetting will not stop bouncing. Bouncing happens when a kitty drops lower than she is used to, drops too fast or drops too low. Letting a kitty get used to normal numbers again is the way to help stop or slow down the bouncing. Bouncing is normal especially in a newly diagnosed cat.

Looking at her SS she was actually doing quite well for a newly diagnosed diabetic cat until you “rest “ the dose.

I would increase the dose back to 1 unit again and hold the dose for at least 6 cycles unless she drops under 50 in which case you would reduce the dose 0.25 units.

 Right. I shall follow instincts (and better advice) next time. Hopefully I haven't done too much damage in the last 4 days. I won't say which fb page I got this advice on but I will take it with a pinch of salt next time.

Thank you both.

I've topped her up an hour after her initial shot - couldn't bear to leave her without what she needs till next shot

I am sure Hercs was diabetic for many months before I ever detected. So he must have been in very high numbers for a long time.
You are doing the best you can! I am sure these 4 days will be recovered!

Elizabeth, we don’t recommend you do that as you will have two nadirs. Make sure you test during this cycle.
You will need to give the next shot an hour later than normal, then work your way back to the normal shot time by 1/4 hour a cycle or 1/2 hour once a day.
I know you are concerned but she will get back on track.

Hi, I'm conscious of this. I'll test her regularly this cycle and into the next which I will give late and creep back over the next few days. Thanks.

I know you are only wanting to do what’s best for Lily and I know how hard it is to see them in higher numbers.
I would follow the TR protocol and you will start to see good numbers again soon.:)

Thank you Bron. Just overthought it because I'd just started thinking I could see her patterns and she'd got wonky so I panicked, especially as I'd got her doses wrong. She's stopped gaining weight (unsurprising with higher numbers), and she's very grumpy and unsettled and has taken against my other half in the last few weeks. She bites him every time he strokes her and will only sit with me where she had always preferred him before. I thought her only wanting her mummy on top of the high numbers might mean she was really poorly but of course she's had a bumpy month so is probably just not feeling the best.

I'll stick to the script and I'm sure she'll fall back in line!

Post each day so we can see how she is going. I’m sure she will feel better when she gets back into better numbers.
I would always get a before bed test in during the PM cycle as cats often drop lower at night. If she has dropped at the before bed test, you might need to get up and test again later to see how low she has gone.
Until she gets used to low numbers you will see some bouncing, but remember it is normal, especially in newly diagnosed cats.
She looks a beautiful kitty.
