I increased Goma's dosage to 1 unit this morning. As noted in her spreadsheet the bg numbers are rising. First 629 after 2 hours and now 746 after 4 (highest they have ever been) What should I be doing now???

On Separate note I am trying to donate to Peanut's cause (and others) but the fundraising sections refuses all my credit cards. Don't know why, they are issued stateside.
 
Are you making sure there is no food for 2 hours prior to each BG test? And making sure that Goma is not sneaking food?

Okay - I am totally confused about all this now...I thought you could only not feed her two hours prior to the morning and evening insulin shots.. How can you perform 2,4,6,8 testing if she cant eat for two hours - This has me totally at a loss.

You've just started this dance, it will take a bit for Goma's body to deal with actually having insulin in her body. Since her body has needed it and hasn't had it, her liver is sort of panicking and compensating by releasing extra sugars. This isn't abnormal, the liver will begin calming down and stop doing it so much. I know it's hard when you see even higher numbers but be patient.

Insulin is a hormone the pancreas should be secreting and isn't - it's not a chemical medicine like a thyroid med or pain med. A lot of things can affect numbers even when you find the right dose.

Additionally - this morning I performed the amps bg without food for two hours. I then fed her and gave her the insulin. When I saw her acting strange, I took a test after two hours (no food) and the reading was 629. I did in fact feed her at around noon and performed the bg test an hour later reflecting the higher test result. Goma is being kept in a cage away from any food source( as there are other cats), other than what I provide. I have been spending a great deal of time with her and bring her into my home office to roam at will.

That's correct.....the only time it's really important that they don't eat are the 2 hours immediately before the Pre-shot tests

During the rest of the cycle, as long as it's low carb, it's OK for them to eat while you're testing

I probably should be posting this over in the ProZinc forum and just realized that. Can this thread be moved there perhaps? How would I do that?

Is it safe to give Goma the second injection of 1 unit today.

I don't use ProZinc, and sometimes it can take a little while to get an answer on the ProZinc forum, but I'd say YES....as long as it's been 12 hours since the last injection, you can shoot the 1U dose again

ProZinc doesn't have a "depot" like Lantus does, so after 12 hours, the first shot should be entirely out of her body.

You don't want to increase too fast, but you're right that you need to get her numbers down. I'd wait for the ProZinc users to advise you for sure, but I think you could increase in .5 unit increments until she starts to show some much better numbers.

So I guess you did see where I posted within your response? I deleted that portion. Not sure how I did that to begin with. Is it safe to give Goma the second injection tonight with the new dose level after all this? How long can her body handle such high glucose numbers. Should I expect to see this drop after the morning injection begins to wear off or will they now continue to increase. Should I go back to the lower dose?

No!! .....you would go ahead with the 1U dose tonight, but I would like to start seeing at least one test on the PM cycle every night....even if it's just a quick test before you go to bed.

Most cats go lower at night, so it's always a good idea to make sure they're safe overnight.

If she's still running in the black's tomorrow, I might suggest going up to 1.5, but see what the ProZinc users have to say. Until we start to see mid-cycle numbers in at least low yellows, I'd increase every 4-6 cycles

It just appears to me that the numbers were gradually dropping on the lower dosage. She did have a pain killer today around 1130 along with fluid therapy. I also just started giving her Felo Form, Glucosamine and anitol (sp) at the vets recommendation so he can stop giving pain medications.
I'll give the scheduled 1 unit injection around 9:15 pm my time (0715 east coast time) and then check her levels in 3-4 hours. I did check bg at 5:15 pm with her not eating for 4 hours since I have no appropriate low carb food. Her level had dropped to 592 as indicated on the spreadsheet. I'll hope to see something in the PMPS test. She will not have eaten for about 3 1/2 hours.
 
You are getting good answers from other helpers. Food at anytime can raise BG. While it is inconvenient to withhold food before a test, it does make it accurate. Yes, it is more important for that food withholding to be for the AMPS and PMPS.

Appreciate the response. I am still trying to get a low carb canned food. The vet continues to push dry food and Hills m/d. I have tried to explain over and over and over and , well I know you and everyone else gets it, he just doesn't. I am changing the feeding schedule to make room for tests when appropriate. After the starvation Goma went through at the vet she is not going to go down that road ever again...
