Wow - it is getting better -Approaching Remission?

Thank you for the support all, lately Wendy&Neko... I took your advice and have been lowering Simba's dosage and his BG readings have come down pretty nicely in a short period of time. Hoping to continue this. The Vet says we are aiming for under 200 for 2 weeks in a row. Not there yet but closer than those crazy high to low numbers. Sincerely. D.

With you skipping this morning, then seeing a 67 today, I think it’s time for another reduction down to 1.0 units.
 
Wrote to my vet and we are now on 1 shot a day of 1 unit.. He believes Simba's pancreas could be working enough now that he may be secreting insulin on his own and possibly in the near future take him off for awhile to see how it goes. *Otherwise he may do fine on one shot a day. If not for this board of dedicated advocates like Wendy&Neko guiding me and others, I would not be this far along 6 months later. Just sayin... :^)
 
Your vet is wrong, cats do better with two shots a day. Lantus doesn’t last 24 in cats. Look what has happened lately when you have skipped a shot, which is essentially the same thing as one shot a day. He’s been high 24 hours later. Better tomreuce the dose and give two shots a day.

The vet is right that there is some pancreatic healing likely going on. But the beta cells are fragile and we find it’s best to slowly reduce the dose in order to achieve longer term remission.
 
Q: How am I to split the dose when often he tests too low for a shot or goes NS? I guess I am answering my own question with NS is NS and .5 when his numbers require it.
 
APPROACHING REMISSION...
With the continued low Blood Glucose levels in the evening I am more convinced that the insulin is lasting longer than the 12 hours anticipated. I'm thinking to decrease the insulin to 1/2 unit and continue only giving it once a day. I am suspicious that the high blood sugar levels in the morning are a rebound from the very low levels the day/night before. If it is still going too low at 1/2 unit once a day then I will most likely consider stopping the insulin and see what the blood glucose levels are at that point. If we are really lucky he may be in remission soon!
 
I agree, 1 unit is too much insulin. I am not sure just one shot a day will work. Look how high he has been in the morning after skipping the PM shot. I think that is from the morning insulin action wearing off. We have seldom seen once a day shots work, unless kitty is on a much smaller dose of insulin. However, things are looking very promising that Simba will get there.

Don’t rush going off of insulin. You want to give Simba’s healing beta cells as much support as you can safely. Most people go to 0,25 units and often even 0.1 units before going off of insulin. We want to ensure that any remission holds.
 
I'll have to see if I can find syringes with more than just 1.0 and 0.5 measuring steps for injection - if they make them... I am not going to hurry to take him off insulin. Like you say - we want to stabilize his early access to healing his pancreas that looks like it is working some now.

His numbers now are all green and blue so NO shots are required.... This is wonderfully frustrating what to do at the moment? Hmmmmmmm Only qualified for 2 shots in 3 days!

Any way possible you can get some additional mid-cycle tests? +3 & +6 both AM & PM would help see the big picture.
 
I am down to giving him micro amounts of what might be +.25 or less - a bit of a dose since the .5 once a day seems to take him too low - Imagine how happy I am getting now. I went more than a day with no dose when he was +400 just to see what it would do and it stayed the same 12 hours later. Now with .2 he is all blues and greens. Need more time to tell.
 
In the Sticky Note on Fine Doses, there is a picture of a dose we call 0.1 units. Might be worth a try if you are still having to skip dosing with 0.25 units. This is real progress. :D

I have some HF calipers and trying to measure the tiny dose now. . . :)
 
1 drop is an easy dose to measure too:
Push plunger all the way in hard before inserting into vial, then release. When giving the shot, make sure to hold plunger pushed in hard while you withdraw needle.
Congrats on the reductions!
 
Thanks - That is about it. Tiny amount - Calipers are nearly useless - know I have tried with these old eyes and u100 syringes in .5 measurments
