Worried about using Budesonide on food-controlled kitty

Hey all,

My kitty has been food-controlled for seven years. Earlier this year, he was diagnosed with stage 2 CKD and IBD (via an ultrasound). I've been trying to manage it with CBD and have added probiotics. He's getting fluids every other day, and 16mg Cerenia every day. I have a regular vet and a holistic vet. The regular vet prescribed Budesonide but I'm afraid to use it because of the possibility that the diabetes comes back. I know Budesonide is better than Pred at not triggering blood sugar spikes. Should I try it? Nobody can help me handle his bouts of nausea. He has not been tested for pancreatitis because the first vet said that the treatment is the same as it is for IBD, so I'm not sure if I should get that test. Thanks for any advice you can give!!!

My girl has both IBD and chronic pancreatitis and her vets have never suggested treating the panc with steroids so I’m not sure about that. The treatment for it is usually an anti nausea med, an appetite stimulant, and pain meds. Have they tried ondansetron for the nausea? It may be that the Cerenia is not working well for him.

As for the IBD, have you tried any diet changes? I’m fairly certain that was the cause of Sophie’s IBD flare ups and she has been great (so far) since switching her to a novel protein

I use both Cerenia and Ondansetron if I need to. The problem with it at the moment is that the dose is three times a day and I'm at work so I can't give him the second dose. He hasn't been tested for pancreatitis because the vet said the treatment was the same as it would be for IBD but I can get him tested if you think it's a good idea!!

I had them run a GI panel just to confirm and made sure it includes the spec fPL test which is the one you want to get if you do decide to test for pancreatitis (not the snap test which is less accurate).
I have also heard that budesonide is best for cats with diabetes but since Sophie is doing well so far with diet changes, we haven’t had to go that route yet. I have seen a few people on here whose cats are on it, so hopefully they can weigh in for you (:

My holistic vet wants to do another ultrasound and then possibly try a fecal transplant before going the Budesonide route. Also I’m going to check out that Instinct you’re feeding your kitty. I took him completely off chicken, turkey and beef about three weeks ago and he’s been eating Whole Earth Farms duck and Primal duck. I haven’t noticed any change yet but I thought why not?

The biggest issue I’ve had with finding novel proteins is that even brands that advertise being a new protein, have chicken or beef when you actually check the ingredients  I just looked up the whole earth farms you listed and it has chicken as well!

Okay I AM STUPID. Why did I think it didn’t have chicken???? Aaaaaagh!

Because we trust that the labels are accurate and don’t think to look lol. Can’t tell you how many kinds of food I looked at and got excited about because they were a novel protein AND diabetic friendly, only to read the ingredients and see chicken!! :banghead:

Trying to find food is exhausting. My pie chart is so small now. I tried Rawz and it’s literally the only food he will not eat, which is crushing because it’s perfect for him.

It’s crazy frustrating! Instinct real rabbit and real venison (not the sensitive formulas) are the only two I have found. The rabbit is 6% carbs and the venison is 3%

FWIW, Whispy used budesonide for a while and it did not affect his BG. He is on Insulin and well regulated, but I cannot say that it won't tip you back in to needing insulin. But if the IBD is bad enough and you can't find a food to fix it, you have to do what you have to do. Fortunately, my guess would be that if the BG did start to creep up, you could likely stop the steroid immediately (if you wanted to) and get back into remission, either without insulin or with a little bit for a short time.

Good luck, I hope you find something that helps!

Thank you! My holistic vet wants to try a fecal transplant first but it’s a backup if I need it.
