Wolvi introduction - changing Prozinc to Lantus

My name is Cris and my sweet boy’s name is Wolverine – Wolvi for friends, he is 10 years old and weights 8,520 kg (he is not fat – he is a big cat – norwegian forest cat).

I also have 5 more cats - they all eat dry and twice a day wet food (I have microchip feeders because I have a renal cat too, so no chance for Wolvi to steal dry from the others).

He was first diagnosed in 2019 and went into remission 5 months after, with Prozinc.
At that time Cushing/addison’s disease, acromegaly, insulin resistance and hyperaldosteronism (because he had low potassium but no high blood pressure) were ruled out.
He had neuropathy on his hind legs, and as we don’t have zobaline here I tried to mimic it with methylcobalamin and folic acid and he fully recovered.

In August 2021 he began to have higher numbers again and we went to the vet - he had a litlle high blood pressure but after some routine blood test, echocardiogram and abdomen ecography (to ruled out another problems).
My vet put him on Prozinc again but 6 months later he is not even regulated, bouncing a lot…

He also took for 2 months Atenelol but not anymore as I check his blood pressure frequently at home and vet said to stop.
He is not taking another medication…

He is exclusively with various LC wet food several times a day (6x because he asks for it).

I home test with Contour Next (we tried Freestyle Libre 2 but it didn't seem very accurate when I tried to compare to blood strips values).

By suggestion of @FrostD and also my vet (and back in 2019 his endocrinologist also suggest, if necessary) we decided to try Lantus.

I already bought it (with my vet approval, of course) and I would like to begin it and have your precious help.

My vet said to begin with 1.5U.

Welcome. When will you be starting lantus? A starting of the same dose he was on with Prozinc or a tad less is fine. You want to start when you will be able to monitor closely the first few cycles as some cats have a strong initial response but you won’t really know what the dose will do until the depot builds in 5-7 days. Just leave a little space between the prozinc dosing and the lantus.

I would like to begin on monday because I will be home all day and more or less the rest of the week.
He was with 1.6U of Prozinc so 1.5U of Lantus would be nice? Or do you think that 1U would be better?
Did you saw his SS? He is a very bouncing kitty...

The vet's plan to start with 1.5 unit sounds good. He was getting 1.6 units of Prozinc and seeing some greens recently. Picking a start day when you can monitor more closely is a good idea. On the spreadsheet, put a blank line after the day you stop using Prozinc and label is something like "start Lantus" so it's easy to see the change.

You should change your signature that you now using lantus
and at the top of your spreadsheet and please put a blank line above the date you started lantus. You can just put started lantus and the date
 
You said you started lantus on 07/02? Sorry I'm just confused about the date
What do you mean ?Can you put month, date, and year in your signature
Can you also please add that to your spreadsheet with a blank line above the date you started lantus so members can see that, Thank you :cat:

just to confirm - should I give the 1.5U for his initial dose and first shot with Lantus tomorrow morning (7th february)?
With prozinc today at PMPS + 5 - 82 and I just give him 1.2 (25% of the usual 1.6U).
He is calm and sleeping, he never eats during the night, his last meal was at PMPS +3
