Can anyone point me to anything stickied (if there is something) that explains what the dose should be when switching from Prozinc/Pzi to Lantus or Lev? What are the "considerations" that come into play? Time on the P insulin, time since diagnosis. etc? Do you start over at a lowered dose, or is the dose close to the same as it was? If you start too low, how soon does that become evident? I know ECID, but are there some general "rules"?

Thanks all,
Carl

Generally, when switching from one of the Ls to the other, we recommend that the dose be reduced by 30%. Some cats have an initial, marked response to the new insulin but this is not always the case.

With PZI/ProZinc, I would probably consider the same guideline. If the cat is on a TID dose, I would think about this as 30% less of the entire daily dose that is then split into 2 doses for Lantus or Lev.

You might want to send a PM to Jill with this question.
