What could be wrong with Smokie's ear?

Smokie, my 17 year old diet controlled diabetic with CRI has been walking with her head tilted a little to the right side and the ear on that side is in a strange position.
I've been trying to look at the ear but she is fighting. I don't know if it is sore but she doesn't want me to check it. She seems a little agitated when walking.
Nothing else seems wrong.
I have got to look in that ear!
Could it be a bug in her ear?
Does anyone know something i could do for her tonight?
 
is she off kilter at all? out of balance?

Mousie once scared the bejeezus out of me. i got up in the morning and she didn't come for her test. finally found her and when she stood up she either fell over, circled, or veered to one side if she could stay upright. first thought was hypo. nope, bg in the 300's. ultimately found out she had an ear infection and as soon as the vet removed a bunch of goo off her eardrum, her balance and tilting improved dramatically. it wasn't 100% til a few days of antibiotics but the improvement with that goo off her eardrum was huge
 
She is walking ok. no problems with balance.
Sometimes now she will shake her head.
I'll bet it did scare you when that happened with Mousie. Maybe Smokie has
the same problem.

Just went through this with the drooler, he was shaking his head, holding his ears at a funny angle and had a bunch of black gunk in the ears....After a trip to the vet he was dxed with a yeast infection because in his case it was severe. But the vet did tell me that once we get it cleared up, because this is a chronic problem for him, that I could maintain it with a mixture of Hydrogen peroxcide and white vinegar, but I don't know if that would work in a cat.

So I would second a vet trip, could be earmites or an infection or just dirty ears but it should be looked at to see what it is before trying to treat and treating the wrong problem.

Thanks everyone and thanks for advise to take Smokie to Vet. I got back a short while ago. She did have a bad infection in her right ear and the left ear was infected also.
He washed them out good and there was a little blood there.
She had her teeth cleaned also and he gave some fluids and B12.
She got all her blood work done.
When i got her home she walked around like she was kinda po'd
and then she went straight to her food bowl and ate a lot.
Vet said the CRI is taking a toll on her kidneys, but i can't let that
get me down right now.
