We have started Cabergoline

Charlie has Acro and has gone on Cabergoline every 48hours since 14 Jan. Do you think he is ready to do every 24 hours? His numbers are not budging and we needed to start him off slowly due to IBD and not upsetting his tummy to much.

Is he doing all right on the every other day dosing? How’s his tummy? Diarrhea? Anything else? Still eating? If you think he can tolerate it, you could try going to every day dosing. Of course, you can always back off if need be. I totally un your anxiety! Some cats take quite a few weeks to start seeing reduced insulin requirements. I hope your Charlie will respond quickly. What did your vet say?

I’m just worried as his BG is so high and has been since September. I know it’s a slow process but it must be causing him some pain.

soft poo and a bit smaller but no diarrhoea.
He is on antibiotics for UTI that will finish on Monday
 
Which brand of novel proteins are you feeding? I see it raw, but was wondering if manufactured or you are making it. I'm just checking if you are feeding him low carb food and whether there's a way to get his dose increased any faster until you get to a better dose. I see you are missing a good number of the preshots tests. We strongly suggest getting preshot tests for each and every shot, to make sure it's safe to give insulin. With Levemir, it's not unusual for a kitty to nadir around preshot time. And when cabergoline does start to work, you'll have to be vigilant about testing to keep him safe.

I'd also either put that you started cabergoline and the date in your signature, or highlight it somehow the spreadsheet so it's easier to see. As Suzanne said, you can try every day and see how it goes. What dose of cabergoline are you giving, and how much does he weigh?

It is a home made diet through Veterinary Nutrition Group. It’s specifically made for he’s needs and has added multi and antioxidants, taurine, choline, Caltrate, psyllium husk, red krill oil and a little greens (spinach or kale). He has put on weight and now about 5.7 kilos. No manufactured food. My Sundays are full of prepping food for him for the week.

So the soft stool could just be from the antibiotics?
 
I hope doing the daily dosing will help. I see he is 5.7 kg now so his dose should be 57 mcg. (10 mcg per kg.) So he’s just a tiny bit low in his dose — so I definitely think the daily might help. I feel sad to see how high he is running. I think the Levemir may really be able to help him. If you can get in the morning and evening pre-shot tests to make sure it is safe to shoot, and can get another test in somewhere mid-cycle then you could follow the Tight Regulation protocol where you could increase his dose faster to hopefully get into better numbers more quickly. My cat went way down in his dose on Cabergoline - from over 40 units. I have a friend whose cat is on Cabergoline now and he is still requiring 19/20 units, but his numbers are excellent in blues and greens on his spreadsheet.

thank you for your advice we have a zoom meeting with a specialist in Sydney Dr Amy Lam so hopefully she can also make some more suggestions.

we are currently outside doing gardening. He can’t jump the fence anymore so doesn’t require a lead and he get sick of his cat run.

Beautiful! Why can’t he jump? Diabetic Neuropathy? If so then he needs to take Zobaline or it’s equivalent methylcobalamin/folic acid supplement. Maybe you are already doing this? Or is it arthritis? That’s not uncommon in acro cats. You could try Cartrophen injections.
 
We can’t get Zobaline in Australia. :(

yes it neuropathy so have just started B-12 Methyl it has taken me 2 weeks to find it and only low dose of 1000mg. He is walking on his hocks.

I’m glad you have started the methylcobalamin for the neuropathy. I couldn’t remember if you said that you had, and I didn’t see it in your signature so I wasn’t sure.

He is very feisty, although is a lot more calm since the health problems. That was part of the reason I knew something was wrong. He would always attack my feet for food.
