Vet wants to increase to 2 units, is that too fast?

Looking for advice on lantus dosing. The vet started her on NPH and I kept pushing trying another. She wanted her to do an in office curve a couple weeks ago because she didn't believe my numbers. When her's matched mine basically (she wouldn't eat there) she finally reached out to a cat only vet colleague who suggested lantus and fancy feast/low carb. Vet was hoping to manage on her current diet due to history of bladder stones and poultry allergy. So Millie is now eating low carb foods, her favorites are the weruva brands. Anyway, the vet wants to jump from 1 unit twice a day to 2 units twice a day of lantus. That seems like a really big jump. Her sugars are fairly steady through the day as you can see on her spreadsheet so you can see they are still pretty high.

Please read the dosing methods for lantus and choose which one is best for you
Depending on which one you choose will let you know when to increase or decrease the dose
Do you have the U-100 syringes with half unit markings, we don't increase or decrease by whole units, also you need to be testing at night after the PMPS cycle, you are only seeing half the picture

I'm not a lantus user so I'll let someone else comment, but I do think you reduced too much when you changed insulins.

With those yellows in there, we would increase to 1.25 units next. Actually, we'd even hold the dose for 7 days to start to build the depot. Today is day 6.

Any chance you can get some tests after the PMPS shot? Maybe just before bed? Many kitties go lower at night, and we make dosing decisions based on how low the dose takes the cat, hence our interest in those night time numbers.

Actually today is day 10 if you look at the dates, I am not able to test everyday. And no, not really possible to test after night dose, i go to bed about 30 minutes later because I have to get up very early for work. My husband is fine with giving the insulin but doesn't feel comfortable test blood glucoses. I think the vet isn't very familiar with lantus at all and seems to only use NPH. It took another vet to suggest it to finally switch to it.

I hope your husband can eventually get more comfortable with testing. It’s potentially dangerous to shoot and them go to bed 30 minutes later. Any chance of adjusting shot times to make it easier to get a test before bed? Or can you set an alarm and do a quick test and go back to sleep? Otherwise get hubby to “help” you test, i.e., watch, help with treats, etc. so that it becomes not a big deal and I bet eventually he will be able/willing to test.

Mine was like that too. Took him a while to feel comfortable doing it. Then he started telling me what I was doing wrong. :rolleyes:

How many hours after the night time shot do you get up? Might be some value in getting a test then.
