This is a cross post from the Lantus forum. I'm really not sure how to proceed and afraid she may develop ketones:

Myrtle had her dental today. Her BG was a bit high this morning at 21.1 (379.8) so I dropped her off to the vet and brought her insulin so he could give it to her if he thought it was necessary. For some reason he didn’t give her the insulin until 1pm (dental was done at 3pm). It is now 9.30pm, she has been eating so her BG will be food influenced. Is it worth testing her BG with food on board? Should I give her more insulin now at 8.5 hours? Or do I just wait until the morning now?

There's no harm in testing but the stress of the day and a mixed up schedule will give almost meaningless numbers. That of course depends on how much insulin she got.
I would just ease back into her normal schedule. If her times are way off you could move them by 15 minutes a day or just make sure she's not low and let the poor girl sleep it off. No one likes the dentist.

I wouldn't dose again at +8.5...I'd wait till the morning. You can always shoot earlier in the morning if you are up and about before 0930am but I wouldn't give more insulin until at least +11.5 hours and that timing won't help you get back on your normal schedule. Does Myrtle have a history of ketones or DKA? Sounds like she is eating fine so you can test her and just make sure she is eating and getting some fluids (some extra water mixed in her food).
