If a cat goes from BG #'s in the 300's to constantly over 670, does this mean the cat may be insulin-resistant and have acromegaly or another issue going on? If the cat continues to stay in the high 600's, does he have a chance of being regulated on a high dose of insulin?

Do you have on specific BG curve(s) you can post?
Also what is dose of Novolin N.
Can't really answer your questions without that information.
Hopefully uoi are using Novolin N and not Novolin R or the combination of Novolin N and R.
Novolin is Novo Nordisk's trade name for a family of Recombinant DNA origin insulins. They include N (sometimes called NPH), and R (very fast actions) and mixture of the two. Only N/NPH is OK, but not that good, for cats.

Since you say the cat is on prednisoline that will raise BG/require more insulin what you see may be on the "normal" side.

Sorry I didn't make it clear but it's not for my cat. My cat is still doing well in remission.

It's a general question. I am curious if a cat continues to stay so high in the 600's, if there are any treatment options.

Acromegaly is a high dose condition, not a high BG condition. I've seen cats on small doses of insulim have wild swings in blood sugar or be in quite high numbers. As long as the kitty is on low carb wet or raw food, has no other conditions that can cause higher numbers (inflammation, infection, needs a dental), and has reached a dose over 6 units twice a day by increasing safely, then we suggest getting tested for high dose conditions. At that point, you keep increasing until the cat gets to the dose they need. We have acrocats with a wide range of doses currently on this board, some well regulated (mostly under 200) on quite high doses.

IAA or insulin auto-antibodies is another high dose condition. More common than Cushings and it is may or may not occur with another condition like acromegaly. But we've also had kitties with just IAA.
