Unregulated on Lantus-High Numbers

Hi All,
my 9 yr old spayed tabby Kristmas and I are returning to FDMB because she was rediagnosed diabetic 9/12 after being in remission over a year. She was in good control this past January and Feb on 5 units a day Lantus with scores in 100's and 200's. Since May her scores have edged upward despite my slowly increasing Lantus by half units. She now is at 9 units a day and most scores are in 400's. She has been to vet for tests- blood work fine except for glucose, no UTI and has been given two courses long acting injected antibiotic just in case she had occult infection with no effect on scores, heart good, thyroid tested normal, vet cant find cause for this. Was at vets 6 days ago- vet says she "looks pretty good" and to up Lantus as high as 16 units a day. Cat weighs 11 lbs, should weigh about 9.

I dont think cat can last long with such high numbers that she's had for several weeks now. Am very afraid of keto acidosis. She was blood tested for ketones and I use ketostix at least every other day and no trace but lot of sugar in urine. High flat curve -more Lantus just seems to push higher numbers. How do I know she isn't already on too much? Or she needs to be switched to another insulin?

Kitty eats canned low carb food generally Tiki Cat, Cat is eating, peeing, drinking more than usual, poops fine, no vomiting or diarrhea, is alert but tired and doesn't exercise. She preens and is responsive and affectionate but is hiding away at times (sleeping in closet). She is asthmatic too with only moderate control - even on Flovent, Ventolin, Accolate and Cyproheptadine still has some wheezing. She is not on any corticosteroids and vet is researching for medication that might improve control. She is indoor only kitty.

I was testing weekly 4 point curve but have started testing (have Alphatrak) more frequently, she has earned increase in Lantus, just not sure if she hasn't already had too much. Any advise appreciated thank you for your time and expertise, Kathy and Krismas
 
I do not think that yo are dosing too much insulin. In my opinion you have two choices:
-- Increase the Lantus dose. Maybe more insulin is required.
-- Change to a different insulin

My MurrFee is doing pretty good on 15 units Levemir twice daily.

There are some conditions that cause insulin resistance.
Two can be tested for by sending blood to Mich State Univ lab (the only lab in North America that does the tests) The tests are IGF-1 and IAA for Acromegaly and Insulin Autoimmune Antibodies.

The third condition is hard to test for, so these two first. Your vet can contact this lab for a special rate shipping info / instructions.

Also -- Lantus is acidic and can sting when injected at higher doses. Please consider changing to LEVEMIR which is not acidic and a very similar acting insulin. I've been using Levemir for my kitty since 2009 with excellent results.
 
I'm glad you got the links to the high dose testing. We normally recommend that people get those done when kitty gets up to 6 units twice a day.

DId your vet check Kristmas's teeth? Infection is a common reason kitties will need more insulin. Secondary conditions like asthma can make it more difficult to regulate. There are a couple kitties here on prednilisone for their asthma or IBD. Sometimes you just have to increase the insulin to balance the steroids which may be needed for other conditions.

As for type of insulin, Neko was over 8 units twice a day on Lantus and it didn't seem to bother her. We've seen some cats get 25-30 units twice a day on Lantus with no problem. Others feel the sting when they get over 3 units. It's something to watch for. If Kristmas looks like she doesn't like the injections then you might want to consider Levemir.
 
Hi, we are having the same problem with switching to Lanus, good regulation for a few weeks then a gradual high flat curves around 325 at 3.5 units twice. Did you find any solutions? Thanks

Hi Samwillie, welcome. You've posted your question on an old thread from last year. You may get more response starting your own thread in the main health forum or the Lantus forum. Make sure to include what and how much you are feeding. Are you home testing, and if so what meter are you using? Are there any other health problems? This information can help others to help you. I'm new here, and not experienced with Lantus, but many others are and I'm sure they'll be glad to help you.
