Trouble Regulating BG Levels

Many people have suggested I join the tight regulation group. I'm having trouble regulating Hannah Belle's levels. If you look at her spreadsheet (in my signature), the numbers are all over the place. The vet just reduced her insulin yesterday morning. Any advice is greatly appreciated.
 
Hello and welcome to Lantus Land. Take the time to read the starred sticky notes at the top of this forum. According to the Tight Regulation Protocol sticky that most of us follow here, Hannah Belle did earn a reduction when she got into the 40's the other day. Good job catching that low.

If you do want to follow the TR protocol, we ask that you get at least 4 tests a day. In addition to the 2 preshot tests which are required for every shot, we need at least one other test each cycle. Ideally those would be mid cycle tests. If you are working during the day, then an out the door or in the door test at end of day would do, plus you can get mid cycle tests on the weekend. At night, a test just before you get to bed is good. We make dosing recommendations based on the nadir, so we need more data to try to figure out what that is.

I read back through your past threads. I think you are definitely on the right track by testing a bit more. If you are able to get a few more tests in between shots, you'll be able to start seeing some patterns. I usually recommend a before-bed test every night, other spot checks whenever you can (out the door before work or errands, in the door when you get home, and curves on the weekends).

FYI, what usually happens with Lantus kitties is that you work them up to a dose that causes a breakthrough. Then they will often start needing dose reductions. Hannah Belle had a nice breakthrough at 2 units, when her numbers turned pretty much all blue and green. What probably happened after that is that she may have started going low in the middle of the cycles, then bouncing back up before shot time. Because you were only testing at shot time, you assumed that she was staying high all the time, or the numbers were all over the place without logic. I'm guessing that if you had some mid-cycle tests at around that time, you would have seen some lows.

So, moving forward, you caught a 40 and reduced the dose. That's good, and probably what she needed. You'll want to give the new dose a few days to see if she settles into better numbers. Grab whatever spot checks you can between shots, and evaluate from there. Judging from the fast drop from 215 to 107 last night, you might find that she is still going low and might need another dose reduction. Sometimes once a cat starts needing reductions, the reductions can come fast and furious for a little while. Of course that is a good thing for Hannah Belle, but can be overwhelming for you. There are lots of people here who can help.
 
Thanks all. Midday tests are not usually an option with work; the AM test is only about 15-30 minutes before I leave. So, unless there is a problem, she gets a BG test 3 times per day: AM, PM and before bed.


I see a new trend in her numbers with the dose reduction to 1.75. The bounce seems to have stopped, but now each test shows a decrease. I have a feeling she'll be reduced again to 1.5 in the near future. Holding at current dose for now and watching carefully.
