Throwing up undigested food 3 hours after insulin.

Hi everyone. My sweet boy link is 15 and was in diabetic remission for 2 years but showed signs again and I took him back for bloodwork and his glucose and fructosamine were high. We started him on 1.5 iu of Lantus just like last time but this time 3 hours ish after each shot he’s thrown up basically an entire meals worth of his wet food, undigested. He seems fine otherwise and will gobble up a snack or treat right after he throws up and has still had water today and probably kept enough food down to be ok with the insulin but I’m at a loss at what is happening. I called the vet this morning and they said if it happens again call back and bring him in but it happened again and it’s 10 pm. He does not seem hypoglycemic just a little sad :(. Any advice appreciated and I have extreme anxiety so try not to scare me please. Also he grazes on wet food all day and since being diabetic eats quite a lot so maybe just eating too much before a shot?

I am sorry to hear what you are going thru again, are you home testing? actually grazing is not a good idea, he should be on a feeding schedule, 3-4 small meals/snacks during the day and the main meals before each shot, testing before each shot is important, if he eats just before shot it changes the glucose level, have you changed his wet food? if so that can be a reason for the upset stomach I will tag a member that can assist you on Lantus, We are here for you 🤗

Thank you! I have not changed his food he eats tiki cat senior line chicken or salmon. I am unable to test at home I’ve tried a hundred times and not enough blood ever comes from his ear or paw pad. The vet confirmed when they tried the same he needs a needle to get enough blood out. Last time he was diabetic it was so seamless. 1.5 iu at 10 am and 10 pm and he grazed like normal. I did have to change the time to 6 am and 6 pm so it’s right after he eats breakfast. Not sure if that’s affecting it. His glucose was 20.8 on day of diagnosis 2 weeks ago. I tried diet reversal for 2 weeks and started insulin again 2 days ago. I’ve had to skip his insulin dose this morning and he had some broth and a probiotic and a teaspoon of pate (much to his chagrin, thankfully he is still very eager to eat more) so far no throw up. But he has thrown up 2.5 hours after his last (and first) 3 insulin shots. He is in perfectly fine energy and spirit levels and is sleeping sprawled out beside me so all good signs I imagine
 
It is very important to know his glucose level before each shot to avoid hypoglycemia, you might want to consider CGM, (continuous glucose monitor) The Libre, is a sensor, which detects the BG with your phone, if you do decide to use this device is also good to manually test, if the BG is in the low numbers (2 digits) this device has the flaw that reads the lower numbers lower than reality, but testing is necessary, hypoglycemia is very dangerous , and unfortunately you cannot physically tell if that would be case, cats hide illnesses quite well.🤗 ;)

I understand best case is to test before insulin however it is simply not possible in this case. I have spoken with my vet and he thinks it’s to do with how much I feed him. I do let him eat whenever he wants basically because I hate to see him upset and I know it’s a good sign when he’s eating. I am nervous to give him more insulin at the next dose time though. Also he’s been playing with his favourite toy (a pasta noodle) all morning so I can very safely assume he’s not hypoglycaemic
 
Playing really does not mean he’s not like I said cats can hide their symptoms very well, the thing is that if he’s too low to give insulin or to high to do so you will not know, that’s not good, besides you want to monitor how the insulin work in his system through the day, in case he needs an increase/ decrease in dose, you do not want to depend on your vet for that once a month or less, and you are risking not to regulate his diabetes without testing, even humans when they need insulin they test first always
 
I’m not sure if you’re purposely ignoring the fact that I can’t test at home. This group needs to understand there are different circumstances for everyone. I cannot test at home I tried every option last time he was diabetic. Not enough blood comes out. I once poked him 7 times until he hid in the bathroom for an entire day. If anyone has advice for someone who CANNOT test at home feel free to chime in otherwise what you are saying is 100% not helpful.
