Are you using the AT3 as per the spreadsheet, or FSL as per the signature? That 50 on 9/23 meant reduce the dose since you are following SLGS.

I am using AT3, 28-29th I used pet meter as I went to get strips as they didn't have human strips at my nearby drugstore so I just used the pet ones for those days.

I will reduce to 3.75, I just find it's hard to see exactly where 3.75 is. 3.5 units his numbers seemed high? 9/16 he had a low of 70 so that should have been reduced too no?

The thing is I noticed his anemia started in September 2023 and he started acting off, that's around the time I started Levemir. Is it possible he has a allergic reaction?

I've seen reaction to Lantus, but not Levemir. Acting off may have been due to the anemia, or whatever caused it.

That 70 was over a couple weeks ago, we don't generally go back that far. Take the reduction to 3.75 units.

Thank you @Wendy&Neko . I think Lantus is more popular maybe that's why more issues cited (reactions)?

I lowered to 3.75 and ordered callipers because it's really hard for me to be consistent when looking at such small readings. Question, I got the Mirataz as well from my vet as he was being picky a few days ago, he isn't any longer but still not putting on any weight :( My question is Mirataz says to use with caution with kidney issues. Given Theodore has muscle loss, low creatinine and dehydration, would this not be a kidney issue? Are there alternatives that would be okay for long term use?

Also any suggestions on how to help him with weight gain. I just don't understand why it's getting so hard for him when we've had OK control recently.
 
No, it's because Lantus has an acid base that stings some cats. Levemir is created differently.

If you have a general Mirataz question, maybe post that on the Feline Health forum. Did the vet say he has kidney disease?
She suggested kidney diets several times which I never pursued as it gives him diarrhea. He is on dr esleys, bff and fancy feast pate.
I believe she mentioned the kidney food because his UPC but does proteinuria mean you have kidney disease? His creatinine is low and he is dehydrated. I will post regarding that in the other forum.

also general question, in theory could giving too much insulin be a cause of proteinuria and weight loss?
 
Proteinuria can contribute to worsening of kidneys - see article here. See the 4th paragraph for some causes of proteinuria. Insulin is not mentioned. Another source of information: https://felinecrf.org/proteinuria.htm Too much insulin does not cause weight loss, but too little can as the body can not properly process the nutrients in food.

Looking back, I realize there were times I should have reduced the dose but didn't, and now I'm wondering if he went hypo during the night. I'm staying up tonight to monitor, and his +4 reading is already 121. I'll try to wake up around 4am to check again. I'm starting to think that I may have caused some of the high numbers and bounces.

I also can't explain his weight loss unless it's because his diabetes isn't well controlled. Just looking for answers but he has so many issues, unfortunately he was dealt a bad card with his health in life :(

Do you think I should reduce his morning dose to 3.5 units? My calipers arrive sometime tomorrow. Theodore usually has a later nadir.
 
Just checked him now and it’s 18.6 (334.8)… doesn’t make sense how he jumped so high
Sometimes it takes a few cycles for them to get used to a new dose. Shooter is on Levemir by the way. He has been diabetic for 3 1/2 yrs. I also had another diabetic cat on Levemir. He was diagnosed at 8 and lived to be 17 1/2. I would ride out the Levemir awhile longer and see... this is all quite a learning curve. Even after all these years of diabetics I still am learning.

He's also got so many other issues. FIV, regressive FELV, megacolon, anemia, high blood pressure (160 ish), almost full dental extraction onto of his diabetes :( he's also almost bald now on most parts of his body. It's very difficult because I am doing the best I can and it just seems like he's not getting better. The weight loss kills me.

@Heather82372 it's funny you ask, I was questioning that since September 3, 2024...

Symptoms:
-balding
-weight loss despite appetite (boney)
-heart murmur (grade 2-3)
-high blood pressure
September 2024 - 34.6
May 2024 - 23.9
January 2024 - 27.8
September 2023 - 27.9
June 2023 - 12.9
April 2023 - 18.7
TOTAL T4 REFERENCE RANGE:
10.0 - 60.0 nmol/L


Bloodwork had a note at the bottom but I was told no need he's normal.

interval are likely euthyroid. Older cats with consistent clinical signs and
high normal (28.3-60.0 nmol/L) T4 may have early hyperthyroidism or a

Following treatment for hyperthyroidism, T4 results will generally fall
within the lower end of the reference interval. However, high normal T4 may
be appropriate if concurrent kidney disease is present."


Finally convinced them to do the THS, T4, free t4 to get this ruled out. We did it today but now I am dealing with his constipation

I’m told it takes 5-10 days to get results back. I’m a little frustrated about the timeline as it should have been done earlier but anyway but he is eating and drinking OK for now. I believe he’s obstructed, I’ll have to bring him back for manual deconstruction. He struggled this morning going to the bathroom. Yesterday she gave him fluids and thought it would pass because he wasn’t straining but it’s not passing.
